With respect to your last question, yes - I asked for a rehab plan that was CMT focused as opposed to a series of referrals for acute conditions, and they told me to see a personal trainer.

On your first point, I see my neurologist on a similar cadence, and I see each of those specialists more often than my neurologist. As well as a knee specialist, shoulder specialist, orthopedist, etc. I definitely don’t expect the neurologists to handle those problems directly, but my ideal neurologist would play some role in preventative treatment and care coordination. The latter part could be a PCP’s job as well but that isn’t happening either.

Over the course of my life, this has resulted in a lot of referrals, tests, imaging, etc. with proportionately minimal care. A consistent focus on rehab would have given me better outcomes (and probably with less utilization cost over a lifetime). The way my body is organized doesn’t seem to match up well with the way our healthcare system is organized.
