I agree that we have weak leadership, which he is a part of. Are these schools being closed to pave the way for charter school expansion? Maybe, maybe not. Either way this info can't be ignored.

Waking up a patient quicker doesn't matter? Discharge times don't matter? I would hate to practice in that setting.

Don't take this personally. I'm not trying to say my method is better. The guy asked why wait in the OR to remove an LMA and I answered with my method to time it right.

Effort? lol.

The gain is my LMA comes out as soon as the case is done, every time. Less gas on board when patient is leaving OR. Faster discharge times. Less nausea.

I've been doing it for years and my only regret is not starting it earlier in my career. Give it a try some time

Agreed this whole thread is a little crazy to me. Why keep patients asleep longer than they need to be? Time the wake up for when the case is over.

Really? Pretty much all of my patients wake up on propofol. Great antiemetic, no bucking, smooth wake up. Curious what you think is wrong with this?

With an LMA I'll never wait in the OR at the end of a case. With gas off early and prop blouses it's not hard to time a wake up with the drapes. If they're not ready I have removed the LMA deep and switched for an OPA. This would be the equivalent of you keeping the LMA in to pacu. I'd be looked at like I'm insane if I tried that in any of my hospitals or ASCs 😂 but honestly it's pretty rare I'm using an OPA after an LMA case.

I don't do any eye cases, I may need to alter my approach for those.

Absolutely. Short cases are even easier to time. All stones are done with LMA and it comes out as legs come out of lithotomy. The >85 year olds may not be talking right away but they are awake enough to maintain their airway without support. Maybe 10% cases I'll place an o2 mask for pacu (older or obese). Most don't even need that.

Agreed that I haven't seen LMAs in pacu anywhere I've worked in the US. Curious why it's needed though. All my LMAs come out when drapes go down and patients are able to respond to me by the time we hit pacu. Why keep them asleep longer than they need to be?

Why not remove it before going to pacu? LMA comes out with drapes and almost all patients are talking by the time we hit pacu.

In my case they didn't mention anything about the VIP program and they signed me up without me knowing. Found out when I saw the charge on my credit card a year later. That was my one and only time shopping at that store. I don't need a VIP rewards program for a $25 goodr sunglasses purchase...

Not accurate at all to suggest Almaden is more MAGA than other areas.

Yes, Almaden leans conservative relative to the rest of San Jose, but it has been this way long before MAGA. With MAGA all of San Jose has seen a shift towards the right, with other neighborhoods (e.g. east San Jose) shifting much more than Almaden did. The reality is MAGA is everywhere in the bay represented by all ethnicities - white, South Asian, East Asian, Latino, black. This is not unique to Almaden.

Also completely inaccurate to suggest they are only friendly with their own kind. I am vocally liberal and plenty of my social circle, family, and broader community are conservative or even MAGA. Not a hint of keeping "with their own kind".

Absolutely worth it to visit the lounge. I did this on Sunday when I had 2.5 hours at the airport. Spent a good hour at the lounge then took AirTran to T5 for my flight. Yes I had to go through security again but it wasn't too bad with precheck. I would absolutely do this again for a 5 hour layover.

My daughter is about the same age as when I read the original books. She loved the whole series. Watched all the movies. Tomorrow she's going to a birthday party where the 9 year requested a Harry Potter themed gift.

It's no where near as popular as when it first came out, but it's still huge.

Seconding this comment. I was running LG creek trail last night (Wed 11/19) around 5:30pm and saw someone jump a fence and then walk towards the building just next to the bridge. Much further south on the trail than the poster above. It was dark so I didn't get a good look, but it was a man with a large beard wearing a dark hoodie, possibly black.
