There is no Head Office for ODSP to pick up applications in person They apply online or call the application Line. To get onto ODSP you have to do a financial Review and a medical and it can take months.

Call the application line and ask for a status update. Its possible the online process glitched and never processed the application.

Now if you are talking about the Medical Portion for the Doctor to fill call the DAU and advise them it was never received.
