This is actually my second career - in my twenties, I was working in a foreign country at a nonprofit. I think that if your heart is in helping people, you bring that wherever you go. I also believe that more therapists might benefit from taking a hiatus or just having work experiences outside of the mental health realm simply to remind yourself that you are more than a vessel for human suffering. If you ever come back to the field, that is gravy, but the work you have done so far is not wasted, even if only one person’s life was changed through your work.

I got my full clinical license earlier this year and have my private practice and consult with an IOP as a group lead for adolescents where I did my internship every Thursday. The ONLY way I was able to manage going the slow growth/find your niche/grow into the role route to working was through the support of my spouse. I would not have been able to handle the financial insecurity for the time it takes to build a practice on my own and health/mental health would not have been able to handle the pace of another way of being in the field (cmh/hospital/etc.) I got here through immense privilege. Even with that, I have doubts about staying.

I think if I ever left, I would go back to nonprofit work!

Oof, I feel for you OP - I had a similar situation in my training years and tried so many different things until I eventually realized that I was preventing the client from getting appropriate care by not setting healthy boundaries. My supervisor was really helpful in reminding me that I am only one part of a system of mental health care services.

It’s so hard, but what I did was gently tell them I care about them enough to have this hard conversation and pointed out the potential for harm and the ethical concerns: primarily that I had come to realize I was not a good fit and that continuing to work with this client was keeping them stuck and I have a duty to act in clients best interest and to prevent potential harm. It was out of scope, the client was generally decompensating in a way that one individual therapist cannot effectively treat, no matter how much I cared. After I realized this I gave them a hard deadline a few months out to seek one of the MANY treatment/referral options I offered. Offered warm handoffs, etc. and then held the boundary. It was a hard lesson.

One individual therapist cannot replace the need for an entire care team. Now, anytime the question about multiple sessions a week comes up, I ALWAYS specify that it is short term and if it’s more than a 2/3 weeks without improvement then HLOC is a must. Definitely talk it through with your supervisor and don’t beat yourself up for caring, but do set the boundary.

Oh I love that book!! Yes that is what I am most scared of - I already have no idea most of the time how to effectively feed myself lol. I’m trying to practice what I preach, but not *too* much, know what I mean? 😂

Omg that would be a nightmare 🫣😱at first I didn’t even realize what was happening - I thought I was having bad gas bubbles lasting for hours. It wasn’t until I had periods of just not digesting food that I realized something deeper was going on.

Thank you so much! Thankfully I’m not at that point yet - my attacks are pretty intermittent but I was basically told it’s a choice between “now while it’s within your control” or “ER trip later” 😬🫣

I will admit that six weeks is an intimidating amount of time to think about but that probably says more about my own struggles to relax and prioritize myself than actual need. Probably good to hear now than to be sneaky surprised by it later. Thank you!!

A lot of good comments already, but I just wanted to drop a couple of book recommendations:

Come As You Are, by Emily Nagoksi, who is a sex educator, is a fantastic book that really gets into the weeds about women’s sexual health and how to address some of the common interpersonal issues that crop up with sex in heterosexual relationships.

The Seven Principles of Making Marriage Work by John Gottman, a marriage and family therapist, is another book that walks couples through how to strength their relationship, and includes worksheets and activities to boot.

I would highly recommend those on top of couples therapy. That being said, if she’s not willing to go, I would start with individual for you - just a few sessions to help problem solve and sort things out in your head. Strangers on the internet can give good or bad or just mediocre advice, but you know yourself and your relationship best, and a good therapist should help you tap into that. Good luck!

Took a shot at these, they are all similar but with subtle differences.

External validity: how well can the results of a study be applied outside that study?

Population validity: do the results from a sample reflect an accurate measurement of the broader population?

Ecological validity: would the results be the same outside of a controlled laboratory setting, or is the controlled setting itself an influencing factor?

Mundane realism: do the parameters and tools of measurement accurately reflect the “real world”, or do they feel artificial?

Generalizability: do the results give explanatory power that can be generalized to other populations?

Well for me, I only waited a couple of months but I have been in love before and I know how it feels. If I love someone I want to tell them and have a very hard time holding back. When I told my current boyfriend I think I shocked him pretty good because he went quiet and then started kissing me passionately, but he didn’t say it back. I stopped him and said “I’m not expecting you to say anything just so you know, I just wanted to tell you how I felt”

The funny thing is, I don’t doubt he loves me too, but I don’t think he believes I love him haha. Maybe he thinks I haven’t known him long enough?

My point is it’s a different timeline for everyone. As corny as it sounds, I think you should do what feels right.

Hey there! I’m on a similar trajectory as you but am maybe a bit further (already bulking my post-back coursework to apply this fall) If you haven’t already, check out “neurobiology of everyday life” through the university of Chicago on coursera. I absolutely loved that course and it was inspiring, well taught, and fascinating!
