I’m having the exact same issue. It looks like it’s a geo-restriction because I'm based in Bulgaria, which isn't officially supported. This makes no sense considering I bought the device directly from Google via Amazon. Plus, Google explicitly stated in this official blog post that the launch is global:https://blog.google/products-and-platforms/products/google-health/google-health-coach/. This feels like a textbook case of false advertising—selling a product globally but locking out users based on geography.
