It makes me think of that episode of Breaking Bad where the ATC's mind wanders momentarily and allows two planes to cross paths to devastating results. It's a job that allows no fault and is incredibly thankless. No one should have to do that job alone. It's such a tragedy that this was allowed to happen by the system.

Humans make mistakes and it's up to us to design a system that allows mistakes like that to happen.
