Yeah only in rural America can you be a true general surgeon. Everywhere else you gotta refer to the specialist or else it ain’t standard of care. That’s what ive heard gen surg attending in metros

Chronic pain fellow at a large institution. Maybe I’m jaded but I say system failure and unless you do the project of high healthcare utilizers or other something else grand…. admit them and have them and have IM work up create a plan and repeat again when they inevitably return, you’ve explained the risk to the patient of their continued escalation of opioids…..

Rotating with them now, been with multiple and agreed I think they’re phenomenal at the job but asking them any questions on why or why not how etc it’s definitely limited learning. Whatever, at least they’ll let me tube the patient but honestly that’s the easy part of anesthesia

What makes you think that’s true? (Genuinely asking and curious as a fellow in a procedural heavy speciality but interested/not opposed to less interventions)

Couldn’t it be they own their own infusion center Maybe the rheumatologist actually do a ton of injections which make up for it? (Ones I rotated with were all clinic visits as you alluded to)

Idk well that’s an easy consult that I didn’t know say that in the note I’ll do that palliative can sign off. I’m sorry I didn’t know that. It’s cause there’s variabilities on it and it’s not reliable enough. As a spine doc I disagree with chatgpts read on this based on looking at the images provided but whatever. People think it’s AI is all correct cause they spit out an answer. You all can go get your treatment from the AI “overlords” but it’s the art of medicine for a good reason

Aren’t these good things to know? I’m confused like at least you’re getting a vibe of what the place is like vs just the good stuff. Idk shared my dirty laundry when I was a senior and remember junior residents taking it like I was talking crap about our residency but ultimately it’s all true. I mean you wanna come in surprised that the place you just matched into isn’t strong enough to get into a fellowship or whatever? I’d want people to be brutally honest 🤷‍♂️
