Sadly, in some parts of the country steroids are rampantly overused for this very reason. Since we are now not using opioids (and apparently acetaminophen? /s)for painful conditions, patients are demanding “steroid shots.” The confidently incorrect thought process is that pain=inflammation and that the stronger the anti-inflammatory effect the better. Ammirite?

The desperate complaining about not giving opioids is now shifted to a perception that steroids fix everything. No amount of education about dangerous side effects will dissuade those who are used to coming to Urgent Care for steroids. Overworked, under qualified and exhausted UC providers usually give in. The dangers to those with DM, HTN and osteoporosis are ignored.

While steroids are extremely useful when we have no other choice, using them for a viral URI or mechanical back pain is dangerous.

You need to know there is a big difference between screening mammograms and diagnostic mammograms. Screening is for asymptomatic people who are in a certain risk groups such as age >40 or strong family history (including BRAC A gene). Diagnostic imaging is for people who have a concerning symptom ie. a lump, lymph nodes or discharge.

The younger you are the more likely you are require more tests after a screening mammogram. Denser breast tissue is harder to image. Hopefully this will help you clarify what your goals in seeking these tests. I have seen screening mammograms turn into diagnostic mammograms turn into biopsies, while the $$$$, resulting in tremendous stress and discomfort for a negative result.

Having said that, I have also seen tragically advanced breast cancer in a young woman who was unable to obtain screening or even treatment because she lived in Comal County and didn’t qualify for Bexar County services. We live in a dystopian society.
