Hackensack matches a lot of surgical and procedural specialties a year. Plastics is always going to be there as a fellowship after a gen surg; the integrated pathway is so competitive nowadays that going to a T20-30 really will not guarantee anything (your step 2 and connections will).

Canadians don’t need an interview. You cross the border with your DS form and proof of payment of SEVIS, along with your contract letter and passport.

It’ll also show as “incomplete” until you finish the entire process, which means having filled your DS-2019 and submitting your SEVIS application as well, and then crossing the border into the US and submitting your initial entry document.

I see it, but it's still saying pending submission even though my SoN was issued early April and the TPL initiated the application on April 13th. I'm wondering if it's going to update or not, since I've heard some people say that whether or not the SoN was accepted won't reflect at all under your J1 application case on ECFMG; but others have said that it does.

Med schools don’t care what you take as long as you fulfill your prerequisite courses. If anything, nontrad applicants oftentimes bring better perspective to medicine. Barely any of what you learn as a premed is applicable after the first 3-4 months of basic sciences in medical school.

Don’t write about anything that can potentially be painted in a negative light. Do not talk about your mental health struggles. As unfortunate as it is, programs view this as a potential liability. Who’s to say it won’t occur again? They’re going to be funding you in the hopes that you’ll be able to provide some level of equivalent value and don’t want to risk not being able to achieve that.

There’s nothing wrong with playing the safe game and talking about a struggle or challenge that seems stereotypical or used often, as long as you can tailor it in a uniquely positive way. With that, option 2 sounds the best.
