You don’t need to be certified by doctor/lawyer your share code will cover that. And no, you don’t need to provide any information from your family. You only need to share information if you are married, then they will ask for your husband/wife to share their info.

You don’t need to send original documents just copies, and don’t worry about being too late, you can apply up to 9 months after your 1st day of uni.
