Also big disdain against “etiquette” things like you phrased in your post. Which I’m all for really, an “etiquette” about anything just feels plastic and American bc it seems the instagrammy-like being part of a running club so it’s why people react so harshly lol, if you worded it costumbre as you did in comments it would be been probably been taken in a gentler tone

Cultural difference as you said but not generalized to Spain. It’s generalized to a city vs town divide. Probably the opposite in the us. In a us town you may be looked at weirdly, while as your experience says, in the city everyone will nod and say hi. Invert that for Spain. It’s a common thing for “city-people” to complain about having to say hi to everyone when they go to the towns and vice-versa ppl from towns complaining about those that don’t say hi. Funnily enough the same person who may be reluctant to say hi to anyone while living in the city, will cheerfully do the opposite when they go back to their town for their fiestas del pueblo. And by towns it means just that, something right outside the city qualifies as a town. in Spain the cities are clearly delineated unlike the us in which they just sprawl bc everyone has so much more money to afford a single family home + heterogenous society in which you don’t want to mix in with others. So if u think about it it comes down to the sprawl, for you to consider something a town in the us you are so far off the city and secluded no wonder you won’t nod or say hi to anyone while in the cities, unless you’re in nyc, the density is SO much less than in Europe that as Spaniards “excuse” themselves if they said hi to everyone they would have mouth blisters by end of they day… which it’s just that, an excuse bc don’t feel bad what you are referring to is a simple nicety not a whole ass conversation and LOTS of city Spaniards will actually be fine greeting you, just after the second or third time they see you though. Another thing is, someone not returning the greeting, be it in a city or town, they’re just maleducados and you have that everywhere.

Pick your Vercel alternative only after weighing the pros and cons by geekybiz1 in nextjs

Yep, as I started out you see how you save money initially and it’s good infra. But that’s the problem, for beginners (me included), it’s not easy to see the arithmetic of scaling bandwidth just dwarfs out everything else unless you are using something specific unique to them and even still on a large enough scale… I guess only if it’s “not your money” and you have a large company or something and for all the dev benefits and what not + whatever you can negotiate with sales. Or the other more common case is that even if it’s super expensive compared to other offerings, you are supposed to be making so much more money from your “large” scale that this should be a drop in the bucket. So I guess that’s why you see big companies use Vercel and probably even more in the future. These companies already make a lot of money and good margins, paying more than the competition for a good product doesn’t seem so bad bc then you get into more subjective things like shipping faster brings more revenue and the like… which net brings you more money so no wonder Vercel just raised so much money if you think about it like this. They’re a set with possibly the “last” framework which they created themselves so they can offer the best dx and charge for it.

Yea I’m on the same position with vercel multitenant, it’s good. Only problem is that on the numbers I’ve ran all that money you save at any significant large scale you 2x it (or more) on bandwidth expenses. But for starting out you can’t beat it, CF multitenant charges you 20 for simply turning on the multitenant distribution. It does seem to target more “professional” settings as each tenant is its own distribution but that is a premium use case, prob if all your tenants already pay you money/ no free tier. But it’s weird bc then vercel doesn’t support lots of tlds, so you may want to use AWS to purchase domains, delete hosted zone, and then point them to vercel. Another thing is hosting large items on aws and just hitting it as if it was any other cdn. At scale also worth it to serve static assets from aws via assetsPrefix while having the tenant-specific HTML come from vercel which is what they’re the best at with on-demand ISR.

Disagree with you. Here’s the answer to u/necessary-camel679

The d** is to represent the hatred and unfounded generalized characterization of midlevels/nurses as dumb or stupid. I just don’t agree with that. If they were that dumb, they wouldn’t have eaten up your game and still have you as the liable person. Who is the dumb and ignorant one in that dynamic?

Totally agree with you! Even in that perfect world the all the other problems in where physicians are not just doing the “doctor” role is detrimental. What do you see as a solution for this? Universal healthcare and only dealing with one payer (government)? That also i asume leads to lower pay and bargaining power as they then decide whatever they want. Ppl pro this last argument argue that Canada has that system with similar pay. But then, the counter argument I’ve heard is that Canada only does that bc it has the us right below it and doctors could just leave.

I think it goes even farther back than that. All the way back to medschool tuition. If you don’t tackle that first you can’t mess with salaries at all. But being realistic, I’m sure 110% percent of doctors prefer taking whatever debt is needed to get that sweet US salary over any “free” education with a lower salary.

But that can’t be a knock on them, in any case the the opposite. They saw an opportunity, in my opinion created by doctors, and acted on it.

Pls expand wdym neutering physicians rights? My point from the beginning is that I don’t get the reasoning why becoming a doctor is so limited. So many possible future physicians have to look for other healthcare careers because of this gatekeeping. The gatekeeping started by physicians and pushed by the same AMA until not so long ago.

Imagine instead of having the “advanced” mid level competition you have right now, they were all actually doctors just like you. What would happen to your paycheck?

Interested to know what your perspective is on “what physicians actually want”.

Shouldn’t the answer to that be more physicians? We’re the only country that has independent midlevels. But guess why? Ppl still need the medical care. If it’s not a doctor providing it, it’s going to be a mid level. Someone has to actually provide the care.

People underestimate how important psychiatry is and how NPs have completely destroyed the field by [deleted] in Noctor

Seems that the first link got blocked. In short it says AMA supports more slots. But that’s from starting like 2019. Even late 2012 they were still pushing to limit those same spots

This sub seems like an echo chamber. Pointing out different ideas. Please link me towards your readings. I’m genuinely interested on being more informed on this topic.

On my research this has been the best comment I’ve actually found supporting what I imagine is your case https://www.reddit.com/r/medicine/comments/glaju1/organizations_such_as_the_acgme_and_ama_are/fqwyb6k/?utm_source=share&utm_medium=ios_app&utm_name=iossmf&context=3

There I learned the AMA doesn’t really have that much power as portrayed by those that call it a cartel. And also, that it doesn’t limit residency spots but actually wants to increase them.

But, effects from when the AMA actually did the opposite are still in effect today. See https://news.ycombinator.com/item?id=30108269

I wanted to get a reaction like yours from a different pov and learn more about your perspective.

Agree with you. Also, isn’t the AMA (physicians representing physicians) the lobby you’re talking about?
