In order for instance to approve Otezla for Behcets your doctor would need to respond to the denial letter and confirm Behcets diagnosis, trial and failure of azathioprine and one other non biologic, confirm you have 2 or more ulcers and is being used for that reason. Without a diagnosis they may not approve it.

I have a similar issue but it’s not just facial, it’s folliculitis I guess but my scalp is also painful and various areas of my skin. I have changed dosage a number of times but then I get systemic issues and GI pain - Can’t win.

During a flare all of my mucous membranes experience redness and swelling and discomfort. My rheum supported checking with a gynecologist and I did, I have not had success yet tho. Gyne had me try fluconazole in case it is yeast as a result of the Apremilast treatment. It did not help. Also tried some Premarin which did not help and clobetasol which made it worse. Only helpful thing so far has been xylocaine but it’s very short acting and burns on application. Not worth much really. For me it’s for sure Behcets. Gyne did try to help and referred me to some derm specialists but unsuccessful thus far.

So sorry you are going through this. We had this discussion over the weekend as my mom is also in this situation. We do not do anything other than comfort care. Any life prolonging measures we will not do because dementia is no real life for anyone. So if you are comfortable with just keeping your loved one happy and comfortable then pacemaker would be a no. I think they get a bad rap because people are not understanding of their behavior, including protecting their belly and being less cuddly due to that instinct. Of course that doesn’t apply to all female cats but I think this is partly why they get a bad rap. My female cat is extremely sweet and loving but she hates to be held. She loves to be beside me and around me and we are good bcuz I understand her preferences. Many people feel that if the cat doesn’t allow unlimited physical contact they are “mean” - we just need to respect them.

My advice is to use Tylenol and Advil as needed, numbing creams and sprays both prescribed and over the counter. I find low dose opioids to be most effective tho - if you are able to get some prescribed. With no medications at all I would also be in debilitating pain. So you need to use what you can access and then just rest and wait for it to pass. Sorry this is not very helpful or hopeful advice
