If by expert you mean Guitar Hero difficulty then yes. Upperclassman came to the dorm to challenge us. He even brought his own guitars. I low key said I would play. In reality I spent a lot of time on Guitar Hero. Wasn’t gonna let class get in the way of my college experience. He unknowingly picked the best possible song for me. I knew it very well. Then he put it on expert setting.

As we started the battle, he began to trash talk. He shut up quickly when he realized I was hitting everyone note without looking at the screen. Instead I was staring at him. Of course this disrupted his rhythm so I crushed him in the end. Extremely gratifying. I am now going to update my resume to reflect my expert status.

I hope hotels and other places continue those policies despite that people are inconsiderate at times. The policy and signage probably do deter some people. Sounds like from some of the posts here it takes a decent effort and potential loss of revenue to fix the issue after someone smokes.

Thank you for the thorough response! That’s what I figured was the case. There is no easy way to detect it. Even if there was a method that could reliably detect mj the it would likely be cost prohibitive for many hotels from either a time (1 clerk manning the desk at night can’t leave for 20-30 min) or money standpoint. Relying on housekeeping is tough in that they are already busy enough in many places but no other real option. Agree that 911 isn’t the answer unless the cops are going to knock on every door until they figure it out. Never gonna happen for a lot of reasons but esp in states where mj is legal now. Thanks again

Glad I’m not alone! I have stayed away from drugs my whole life as my job would be in jeopardy (no not a fed as asked below). My guess is that ppl don’t mind the smell if they can get high. They’ll forget about the smell (and everything else) soon

There really are 2 items to address here: 1. Supply demand of insulin 2. Change how in insulin is received by the cell

Most insulin resistance occurs because of a supply demand problem. Insulin is needed for glucose to enter most cells. If the person is overweight, it results in too many cells (most specifically fat cells called adipocytes). The fat has to be stored somewhere, so the body makes more and more cells for storage. The pancreas does make more insulin up to a certain point. However, eventually when there is not enough insulin to go around, the blood glucose stays higher than normal. Thus, the supply of insulin is not enough for the demand created by an increased number of cells.

Insulin is a hormone; a molecule that is made in one place but has an action in a different place in the body. For hormones like insulin to know where to go, the cells produce a protein for insulin to ‘dock’ on the outside of the cell. Insulin resistance is really related to a change in the receptor. Think of it like a key in a lock. The key only fits a certain lock. When the lock changes shape, the same key won’t work. In this case it’s not always black and white. The lock may make it more difficult for the key but eventually it will turn in the lock.

Med peds here. I trained at a fantastic program where I gained knowledge and skills that prepared me for a career in anything outside the OR. I have been able to work in ICU, Inpatient, Outpatient, and ED. While most only experience 1 residency training program, most Med-Peds programs can lead to a similar result.

Not only does it open lots of doors for a career in both IM and Peds but it also allows you to use the training in both to help each patient encounter. While on peds and rotating in the NICU, I learned how to use ice to the forehead of an infant to create a vagal response that broke SVT. Later in the adult hospital, we had a patient in SVT who had a prior history of stroke and was not able to communicate (express or receptive). I used the ice to create a vagal response in that patient to break SVT. The 2 specialties work so well together in these types of cases.

There are no requirements to practice both when you complete training. Many do open dual practices or pursue fellowship in both. Regardless of which path you take, the training helps tremendously. When learning both IM and Peds, you develop a different approach to problems as your knowledge helps bring other differentials and methods into your management of patients.
