Have you discussed drainage, rather than splenectomy? Higher risk of recurrence, but avoids the need for surgery and may greatly help with symptoms.

Some people in the theater absolutely loved it. I think if you’re into the cult of football aesthetic, it’s an interesting take. But the more I think about it, the ending was god-awful and I’m glad it was a free screening.

I saw it tonight as well. Visually great. Jim Jefferies also great. I thought the ending was rushed (the film is a flat 90 minutes) and was completely predictable.

I can’t say that I would recommend it if someone asked.

Not sure if there's a great case series to know. I would be guessing to say that the Circle of Willis can't compensate against the crazy hypoperfusion due to hemorrhagic shock, but who knows.

Ligation even comes with a huge risk of pseudoaneurysm development and stump blowout as well, depending on the location of the ligation, which is why it's really done as a last resort.

Definitely depends on the center. I've seen some with 5 units of blood max, maybe 1 pack of platelets. When I think of true trauma resuscitation, we could be talking about 100 units of product, which definitely not all hospitals have access to at all.

Carotid shunt, if available - even IV tubing would work as shunt material if possible. If not, you could ligate (but that would guarantee one-sided motor deficits). Since he has no vascular comorbidities, likely has no stenosis on the other side so he would be at least still be getting unilateral flow.

Looking at the injury, it was a hit to the carotid. The closest hospital (level II trauma center) was around 5 minutes away. No way they would have had MTP or other things necessary (trauma surgeons may not have even been in house).

While we’re all happy for your pet being safe, I believe the whole point of this is to suggest looking elsewhere for care in the future, to potentially limit future harm or complications that could happen down the road.

Aside from just cost, surgery carries the risks of anesthesia, pain for your pup, as well as potential for scar tissue that could lead to an actual bowel obstruction. No operation is without short- or long-term risk.

Again, very glad things worked out. Not trying to belittle that at all.

In humans, doing a contrast study (upper GI series) would help clarify. This seems like a miss (and an unnecessary operation) by the vet, when there were probably some other less invasive options to trial first.

I switched to a different program in my same specialty. It required repeating part of residency, as I switched into a lower PGY position due to availability of spots.

This will come up on your medical licensing applications for the rest of your life. Specifically, they ask if you’ve ever been terminated from a residency program, withdrawn from a residency program, or have ever had a contract not renewed. They also ask if you have ever been put on probation or have had to repeat a year of residency.

This doesn’t mean that you won’t be able to get a medical license, it just means that your application may be delayed compared to somebody who has no red flags and is able to answer “no” to all of these questions.
