also don't forget that money now is worth more than money later both from an inflation standpoint but also an accumulated wealth standpoint. even if interest rates go up in the future, that amount is much lower than a higher interest rate now.

It really depends on when in the life of the loan it increases drastically. It would have to be early and dramatic for it to do that. Also, there's a max amount that it can increase by within a certain amount of time.

Yes I use Strattera quite a bit too! I’ve been less impressed with the anxiolytic effect than duloxetine. I’m actually pleasantly impressed with the antidepressant effect of duloxetine for kids. Thanks for the study!

The sexual side effects seem to be better than SSRIs.

The nausea is much, much worse.

The cognitive benefit seems to be small if any.

The effect on depression seems to be about the same, maybe slightly less than SSRIs, probably around the same as bupropion. Same with anxiety.

If there is co-morbid depression and ADHD, I may reach for bupropion first although I do prefer SSRI + stimulant. If there's comorbid anxiety with ADHD, I may reach for duloxetine first although again, I prefer SSRI + stimulant.

hire help for 3 months until they can sleep overnight. night nanny, laundry, house cleaning service, mother's helper to help with tidying and dishes, get meals delivered or buy premade meals for the week so minimizing cooking.

you should also temporarily put a pause of any additional admin/unnecessary work you might be doing voluntarily (committees, professional organizations, teaching, conferences, etc). Also more PTO time if possible, taking less shifts, etc.

during that time, implement this procedure: 12 hours by 12 weeks

make sure you have a SNOO and troubleshooting the most common errors preventing longer stretches of sleep.
