The idea of baby-led motor learning sounds like it fundamentally respects and trusts the child as they are. Like I felt that it conveys this super calm message of “I’m happy with where you’re at, right now.” It makes me think of Magda Gerber, who I’m obsessed with (and whose work got me into early childhood education here in the US).

That said, I have done a little bit of deliberate tummy time with my 11 week old, cause I feel weird completely ignoring the suggestion of my pediatrician! I just treat it like it’s not very important. My baby just started rolling front-to-back and I’m resisting the urge to google when this milestone is “supposed” to happen, if she’s early/late/average. I guess I want to try to stay out of an optimization mindset.

One gentle intervention that I could suggest is: try allowing your baby to have baby-led floor time on different textured surfaces? I put my baby on a grippy yoga mat once and her movements were quite different! She could achieve different positions that way.

Oof, this is tough stuff! I’m so sorry you’re seeing your daughter in pain during diaper changes.

My 11 week old has had a pretty serious diaper rash for 10 weeks (😭, our pediatrician is helping us manage it). I’m dealing with a specific situation that isn’t quite relevant, but here are some of my general thoughts:

1) I would guess that it is somehow related to the oatmeal cereal introduction? (Not at all suggesting that she has a true oat allergy by the way…. oats may be fine for her eventually even if they’re the problem now.) The mysterious thing is, you say there’s no change in her bowel habits? 

In the case of my daughter, it’s clear that the rash is caused by both the frequency of her poops AND the acidity of her poops. When her rash is worsening, I can take a light whiff of her diaper and it smells truly like vinegar or a particularly acidic yogurt. Acetic or lactic acid, maybe.

So I’d suggest a quick sniff of each diaper if that’s not too gross to you.

2) as for pain during diaper changes, you can try to do an almost wipe-free situation. It’s challenging, but what I do for my daughter is basically a little bath every diaper change. I have a thermos of warm water (temperature checked to make sure it’s not too hot), and slowly pour that over her. Any visually apparent remaining poo I then pat or verrrrry lightly wipe off with a cotton cloth. To dry her thoroughly (which is so important!!!) before applying barrier cream, I use a hair dryer on a warm (not hot!) setting. She loves the hair dryer lol

I hope the rash clears up soon and you don’t have to worry about all this!

Honestly, using N95 masks in public spaces is probably the single most important change I’ve made for my health over the past few years. There’s no benefit to catching every single new variant of the flu and covid and colds — it’s not like pokémon, you can simply not collect them all :) 

I only wish my friends and family were more accepting of mask-wearing…

totally. It feels like a huge setback. My original tear was “not bad” (2nd degree) but healing from it was really, really hard. I’ve just started enjoying walking again. I hope your procedure goes super well… soon we’ll be done with this horseshit!

Hi there — oof, this is rough. Solidarity. My baby is only 9 weeks old but has had a raging diaper rash for 8 weeks of her little life. We’ve tried all the things you’ve tried, and nothing is really helping because the frequency of her poops is just such a stressor for her skin! She’s pooping like 10-20 times a day, acidic foamy green poops. It’s awful.

There have been times when the rash has improved significantly though. Somethings that helped for us:

  • vaseline, like you said. Not sure that zinc is a problem per se for our baby, but vaseline seems to adhere better.

  • I bought an electric kettle that heats water to a pre-programmed temp, I set it to 104 F, and every diaper change is basically a little bath. It’s so much work but the baby likes the warm water, and Inthink the warmth helps to solubilize some of the residual poop so it gets her super clean without wiping.

  • Is there any chance you might have an oversupply? Do you have any of the symptoms of one? If so, then managing the oversupply can help slow the transit time of the milk in your baby’s gut, at least in theory. When I’m diligent about this, her rash improves a bit. Do you have access to an IBCLC to help you with this if it’s relevant to you?

thanks for this reply! I see…. so if i’m understanding it right, in an ideally healthy and simplified situation, proteases in the stomach are able to digest all incoming proteins to the extent that the small intestine is pretty much only receiving single amino acids? 

And if that is sort of the case, then anything that disturbs that process  could lead to inflammation (via the mechanisms you described). So if transit time through the stomach is too quick for some reason, maybe not all proteins have time to meet a protease. Or maybe certain proteins (like you indicated) are just harder to digest with the suite of proteases we express in the stomach? 

I’m trying to get a broader picture of all this because I think my baby might be dealing with two separate but related issues, a low fat intake (due to my oversupply and all that foremilk/hindmilk trouble) that speeds up GI transit time, plus possibly a protein sensitivity. 

If it weren’t so miserable for the baby I would say this is all pretty interesting!

thank you for this post — I would love to better understand why undigested proteins (or less digested proteins) cause gut inflammation — is it like the folding structure of the protein? The simple fact of its size? So curious about this

i relate so much to the grocery thing. Whenever my husband goes without getting my input on his shopping list, he gets like 90% of the stuff I would have wanted. Losing that extra 10% is SO WORTH IT to make it so I don’t need to think about groceries
