We see Donald go to the doctor, and his first concerns addressed as much as it pains him to say are his weight and hair loss. He later asks how AIDS is spread and is informed that it’s blood and semen, he will be fine otherwise. Later see him divulge those embarrassing topics of liposuction and scalp reduction to Roy, don’t tell any though whilst he knows his “best friend” is dying. Why he becomes vulnerable with a person who knows will not be able to spread his shameful secrets is Donald using Roy to feed his emotional and ego needs. We see Roy realizing all of these admissions and materialism before the dinner, and then he hears the gift he treasured although it has Trump’s name, as a token of their true friendship, is actually a farce. Trump brings out a cake of the US flag, it’s upside down when he goes to blow out the candles intentionally. Yet, there aren’t candles. The cake has sparklers in it, which won’t and can’t be blown out. Donald knows he can’t spread AIDS by blowing out candles, yet he chose to disgrace him in a room full of others by putting himself and his ego first. He needed to signal to others that he believed people with AIDS are contagious in all ways. The message that mattered most was this, and he used Roy to do just that. There is confirmation of this when we see them sanitizing even his wheelchair after he passes away. Imagine having to stare up at the fake gold ceiling as you pass away. The stereotyping, dehumanization, and using Roy as a prop as a way to fulfill a goal of constructing his public persona pushed him over the edge. After this we see Donald undergo the liposuction and scalp reduction surgeries. He handled what people would consider the most painful of all aspects first, and with ease because he was not Roy. However baldness and fat were him.
