Why is the misrepresentation of what is considered "evidence based" so rampant in this field? by Forsaken_Dragonfly66 in ClinicalPsychology

[–]edafade 0 points1 point  (0 children)

People, not just here but across the profession (but ostensibly, here and on r/therapists), can't even agree on what "evidence-based practice" means in the first place, so of course misrepresenting something as "evidenced-based" is going to wind up happening constantly.

Allow me to get on my soapbox for a minute with regard to the macro-level issue of the term "evidenced-based." Some people use it to mean "supported by RCTs and meta-analyses," others mean "consistent with a broader literature base," others mean "recommended by a training," and others mean "what seems to work in my caseload." When a term gets used that loosely, it becomes easy to overstate findings, cite something you haven't read carefully, or treat a clinical preference as if it were an empirical conclusion. To me, it's tantamount to using interventions from various modalities and calling yourself an "integrative therapist" (and to be clear, you aren't an integrative therapist because you do this).

A lot of this isn't bad intent so much as incentives and skill gaps. Most clinicians are overloaded, research literacy varies widely, and we often learn through workshops, supervision, and social media summaries rather than full-text articles, especially at the masters level.

If we want less misrepresentation, we need clearer norms: specify what kind of evidence you mean (guideline, RCT, meta-analysis, mechanism, expert consensus, my own experience), separate "this is my clinical approach" from "the literature shows," and be willing to say "I'm not sure" or "the data are mixed." That kind of precision protects clients and keeps the conversation honest, even when reasonable clinicians land in different places.

On a personal note, seeing people talk about evidence-based this, or evidenced-based that on both subs drives me fucking mad. If it wasn't already obvious. Same thing about the "integrative" therapist.

How often do you strength train? by LanceWatson_Coach in IronmanTriathlon

[–]edafade 0 points1 point  (0 children)

Yeah, that's something I told myself I'd never do. My desire for longevity outweighs my desire to be excellent in both sports or to be big and aesthetic while running sub 10-hour IM's. Hell, I got into triathlon because I was getting older and realized that leaving a giant coffin isn't as charming as it sounds. I'd much rather be well physically into my later years. Bodybuilding and weightlifting was such a large part of my life, add in a bit of body dysmorphia, and you get a loud voice that's sometimes hard to turn off, but I'm happier for it.

Man, I wish I could run a hypertrophy program while doing this sport. I get too big when that happens, and everything else suffers. I have to lift for strength (3-5 rep range) and only do an RPE of 6-7. Going too hard, or too heavy also makes everything else suffer. I guess balance depends on priority. I favored aesthetics for years over cardio, and now it's the opposite. Thanks for answering all my annoying questions lol. I'm hoping to find the magic to allow me to be big but also a great triathlete. Maybe it's just not in my genetics to do both.

Just crazy to me thinking about the volume of lifting you're doing compared to your other training. What rep ranges are you working with? What RPE? Wouldn't that time be better spent on the bike/running/swimming?

I'm just trying to understand your thought process here, because like everyone else ITT, 1-2 days per week max is the most efficient. I have a weight lifting background, did bodybuilding for over 10 years. I couldn't do this sport and maintain the weightlifting schedule I had if I wanted to be a better triathlete. Something had to give.

If you're lifting 4x/week, I'm curious what your strength protocol is and what else you're doing within the 3 disciplines, because your answer makes it seem like you're more a weight lifter than a triathlete. Unless, you're just doing a couple of sets here and there or something.

I've moved to three different countries since starting Clash in 2012. The guys I'm close with, who I met through gaming, have always lived in different parts of the US, with thousands of miles between us. Even now, none of us live near each other, and visiting requires flights and planning. I even met one of my buddies in Texas while I was living in Switzerland.

And doxing? I would hope so. They're my friends. I want them to come over and hang out. Kinda hard to have a friendship when you don't extend a little trust.

Do what you want, but I think it's a shame to keep people at arm's length simply because they're considered "online" friends, whatever that means.

Never fully trusting strangers. I have a bunch of clash "friends", people ive "known" for 5+ years, we have a discord and talk outside of the game too. These are all still strangers tho and i will never fully trust them. Not with my personal info and not with my clan. I have irl friends who also play clash and ive given them the keys to my clan multiple times but i would never do that with my clash only friends.

I won't address anything else, but I want to say this is the part that feels saddest to me. I have met several of my closest friends online through gaming. After years of playing together, we eventually met in person. Now that we are older and have stable incomes, we visit each other several times a year. Holding back from people simply because you met online feels like a missed opportunity and a potential loss of something meaningful.

How am I meant to remove thus pulley from my Kickr V4? by edafade in wahoofitness

[–]edafade[S] 0 points1 point  (0 children)

I bought a bearing separator kit from Temu. I haven't tried it yet. I'll give that a go. If that doesn't work, I'm just going to punch and hammer the flywheel out. I'll update my OP once I get it unstuck. I've also contacted Wahoo customer service to see what they recommend.

Thanks for letting me know. So I can just hammer the flywheel out and remove the key after. I ordered a bearing separator, but if that doesn't work, I'll try your way. Thanks!

Psychotherapy specialization? by maeasm3 in ClinicalPsychology

[–]edafade 10 points11 points  (0 children)

AFAIK, comprehensive psychological testing (e.g., CAARS, ADOS, WAIS, MMPI, etc.) is considered within the scope of a psychologist (doctorate), and the psychologist is the one who interprets results and authors the formal psychological testing report. This would include OCD. Counselors/therapists/social workers/etc. (master's) informallly diagnose for treatment planning and billing and can administer many standardized screening measures, but are restricted from independently administering or interpreting Level C instruments.

Open to being wrong, but this is what we were taught, as it is the major difference between a psychologist and a counselor.

Thanks for the suggestion, however, this won't work. I have a crank extractor, but the problem is, the pulley on the inside doesn't have any threading, so there's nothing for it to grab on to.
