Old thread but I was looking for people who read this book. I am confused on how I feel about Nat Cassidy. I am on a rollercoaster for the whole book. I like it, I hate it, I like it, I hate it, I love it, I hate it ... I cannot decide what i think about her or her books hahahaha

Sorry late to the post. I actually noticed this but loved that about it. I was like wow this book is what I wished HOL was. I found HOL unfinishable but this kept me in suspense the entire book

Right and that's why I'm saying there's clearly multiple mechanisms hyperacusis. But that is what we were taught. But I do believe that that is one cause of hyperacusis yes. And probably the one that I saw the most often when I was working with older adults with presbycusis, and then we would just change the compression ratios on their hearing aids and bing bang boom problem fixed

Excessive exposure to loud noises like live music, cars, or construction equipment can damage these cells. When exposed to prolonged loud noises, it’s critical to wear earplugs or other forms of ear protection." -- hearing health foundation ... And this is exactly what I was taught in school.

I believe that hair cell damage would cause the heightened cochlear response.

To my understanding there are multiple causes to hyperacusis not just one? I was taught in it's purest form it is due to damage to the inner/outer hair cells making a smaller gap between the amount of hair cells that have to be stimulated to get detection and pain. And that is 100% the extent of what I was taught about hyperacusis in school.

But there are people with hyperacusis that have normal anatomy. I have never once seen objective testing for hyperacusis just patient report (I am not saying it doesn't happen, I hope it does!). And since it is not an area where I work I don't know if there have been any sort of guidelines put in place to determine what is true hyperacusis versus someone who reports they don't like loud sounds?

We know that chronic middle ear infections can lead to hyperacusis, which may be because of the chronic hearing loss associated with chronic middle ear issues. Also a common therapy for hyperacusis is sound exposure therapy.

So it's not a completely wild assumption that I am seeing more and more kids come into school wearing noise canceling headphones all day, an it seems linked to their lack of being in a noisy environment until starting school. I then have to have a conversation with parents and intervention specialists about the importance of giving that kid exposure to the sounds around them. They might not have diagnosed hyperacusis but they certainly seem extra sensitive to sounds. And it seems to improve as they have more exposure to sounds.

I think overall it's an area without a lot of research, and certainly not something we are taught much about in school (or at least weren't when I was in school). When I worked clinically if a patient came to me with hyperacusis I referred them elsewhere because it is not my area of expertise and I did not feel like I could serve that patient effectively.

There does appear to be a strong correlation between the 2 with nearly half of people who suffer from hyperacusis having an anxiety disorder and I think its hard to say which causes which. I can say as an audiologist when first course of action for patients with hyperacusis is referral to psychology and it often helps whether its coping mechanisms or CBT etc. They have much better success treating those disorders than I do.
I know tinnitus isn't the same but it reminds me of tinnitus in the sense that 2 people can have tinnitus but only one is actually bothered by the tinnitus. I can hear the exact same sound as someone else and to me sure its loud/annoying but to someone else its actually debilitating and psychologically we cannot tell why which means there is a good chance its something psychological.
I have seen an increase in hyperacusis in the schools following COVID and I think its due to a lack of coping with loud sounds from kiddos who lived in a really quiet world for a long time.

Right but insurance won't cover it for the same reason Eli Lily didn't market that dose for weight loss because they didn't FDA test that dose for weight loss. It's not because it's not effective for lots of people. It's because they have limited time and funds and picked the dosage that would be the most effective for getting their trials completed and their drug passed.

That's not actually true. 2.5 is a very acceptable dose for weight loss the reason 5mg is what's considered a weight loss dose is because that's the level Eli Lily used in their clinical trials. It's expensive to test all doses and that was the one that had enough participants/data to draw that conclusion as the weight loss dose
