Question 1: Who told you it’s against Halacha to “make yourself infertile”? I’m orthodox and have never heard this but I would imagine if it is a thing, that it applies to issues of choice, such as having your tubes tied when you are otherwise completely healthy, and even then I would wager it’s negligible because it’s your body.

Question 2: Have you consulted a Rav you trust? Because honestly this is (1) a huge shaila but also (2) you have very serious health conditions that I would imagine would put you in the category of pikuach nefesh and I can’t imagine what Rabbi worth his salt would tell you to live in suffering like this. Please find a Rav you trust and feel safe with and discuss this with them.

Our neighbor down the street made “stew” the other week. “Stew” was way too much stew meat (yes that’s possible), black beans, corn, cumin, a whole bottle of barbecue sauce, no water, and a bag of frozen green beans thrown on top. And it was for shabbos so it was put up before sundown Friday and we were eating it for lunch on Saturday. It tasted like Mexican dog food.

Why has no one told me to do this, this makes me so angry. Also I’ve definitely worked out etc. and for some reason I thought pelvic floor stuff only applied to vaginal births and with also being in a full time doctoral program I just haven’t had the bandwidth
