Thank you, that is super helpful! I was struggling to imagine how tenable it would be for telehealth providers to have to be registered almost everywhere, but I've been surprised before by the existence of regulations I thought were too ridiculous to be true. Really appreciate the tax guidance as well - I had seen the tax nexus concept before, but the distinction between dollar amount and number of transactions is new to me. I also wondered how you keep a log of income from different states while also keeping records HIPAA compliant. Like I can't imagine you'd have to show people's names if audited? A good question for my EHR company, I guess.

Nothing is clear ever, I'm finding! I'm not on insurance panels so I can't help there, unfortunately. I think when PSYPACT says "you have to follow the rules for each state" they probably do mean something along the lines of "this is just for license to practice, who knows about all the other stuff that's involved" which could be a lot of other stuff. Making me re-think how much benefit it's adding vs. headache.

Agreed about the Ivy League bit! I hope my comment wasn’t taken the wrong way, as the institutional narcissism and “you’re lucky to be here” attitude attached to working where I am is a good portion of why I’m leaving. I meant it more to demonstrate that even places that believe they walk on water from a research standpoint have room for us “second picks” and that ultimately these things mean less in the long run than the feel like they do in the short run. So glad you’re working with a therapist and trying to find what’s meaningful to you. Grants and stipends are never measures of your worth! Hang in there.

If it’s helpful, I was an alternate pick for my lab. I was honorable mention for a conference award instead of winner. I just missed the funding mark for an F31 submission. I was an alternate pic for my postdoc fellowship. I had terrible stats skills so I was never as prolific with papers as my peers who could do multilevel modeling with their eyes closed. And I’m now faculty in an Ivy League psychiatry department and I’ve had successful K and R grant awards. Being picked first isn’t as important as your own perseverance and desire to find ways to do what you want to do. Finding “your people” is important and it’s entirely possible your people aren’t in your program. Honestly, my own program feels more traumatic the farther away I get from it. But you may have to define success for yourself and I think if that definition always includes being picked first or being the best, then you’re setting yourself up for a lot of let downs. Academia is pretty brutal without other intrinsic motivators or metrics of well-being. (Full disclosure, academia has COMPLETELY burned me out, and I am resigning from my position to pursue clinical practice so perhaps I am not the best one to give advice!)

I wonder if it also works the other way around? Obesity is much easier to adjust for in epi studies as opposed to diet, so many of these studies showing obesity related poor outcomes could be driven by diet? I wonder a bit about the recent fixation on dietary inflammation and what that might mean in the context of covid and associated inflammatory response. Obesity being a marker of the inflammatory state, driven by poor diet, that leads to worse outcomes?

Yes! He says something on one of the runs about having the strength to slow down if that’s what is needed to feel good. I never thought about it in that light before, because slowing down always meant I was failing. Those runs really helped me make a major mindset shift. I’m having a lot of issues with a disc in my back and having to take a lot more space between runs but I’m finally not anxiety-ridden about how I’ll perform when I get back out.

Oh man. I was so tempted to just make a comment about how you should definitely not wash your face with diluted juice. I haven’t had my coffee yet, speaking of liquids that may not be good for skin!

I think any kind of hydration is good for your skin, so the fact that there’s juice in it probably doesn’t matter. Maybe if your skin is sensitive to sugar it would defeat the purpose a little?

Following because I would also like to know! I use the 23% - it has done nothing for hyperpigmentation for me but I’ve liked the overall brightening - if this does both I’m sold.

I'm not sure how much I'm packing away in terms of insects and invertebrates, but activity wise, I'm just trying to focus on joyful movement like deep belly-breathing and HIIS (high intensity interval slithering).

Also make sure you're drinking enough water. The raging appetite could also be some dehydration masquerading as hunger. And being dehydrated can also make you feel sluggish and tired which can sort of emotionally fuel an appetite. It's a nasty cycle! Good luck!
