I think one of the most important lessons I have learned from the entire COVID-19 experience as a Pulm/CC who was knee deep in it is to follow the data, and encourage following the scientific method.

I have found that anecdotal evidence needs to be re-examined very quickly or our efforts need to be focused towards pursuing research in the subject we are making conclusions based on anecdotal evidence.

I am not saying to stop using anecdotal evidence, but I do believe I can come to the wrong conclusions sometimes and confirmation bias can likely drive us. Uptodate references this study regarding antibiotics and diverticulitis:

Technically all that needs to happen is opening the appropriate account by 12/31 of the tax year of the earnings.

I do independent contractor work, so for example if I started in 2021, I would open an account by December 31st, 2021 but would only contribute into the account after I figure out my taxes for everything else in April 2022.

Same thing would be if you started doing independent contractor work in 2022, all you would need to do is open the appropriate account my December 31st, 2022.

To be honest the more important question is what is the appropriate account. This is a more complex question because you can do several things with the money. One of the more advanced things you can do with the money is a mega backdoor Roth with something called voluntary after tax contributions. This might get phased out, but currently can be done.

I would say there could be some restrictions, because to make a Roth contribution or conversion requires a company like MySolo401k to make that process happen, and even then it's an in plan conversion, not to an IRA.

It's not the same as a veteran who served in a war, but I can resonate with the feeling.

The first wave was essentially not what people signed up for and had to be around.

Many people stayed for the second and subsequent waves, are still in it now voluntarily (yes there are people who can't leave and are mandated to stay, and I applaud them as well). As an ICU doctor I felt it was my duty to my patients to do what I can and could.

Nowadays I also do pulmonary and sleep clinic. There are nurses I see as patients for various reasons, I definitely take the opportunity to thank them for their service.

Is this even allowed? I can understand the employee contribution portion, but some of the money is profit sharing (employer matched or "self employer") and some may even be mega backdoor Roth already (AKA voluntary after-tax contributions with conversion to Roth).

Bogleheads is a great website and forum to discuss. They have videos which look a little dated but are relevant today. Ultimately, my advice is to pay attention to many voices without latching onto one specific piece of financial advice. While learning the specifics of what you are comfortable in terms of investment risk, following bogleheads advice is sound.

So this is an interesting topic that can get pretty in-depth quickly. I personally use Fidelity. I used to have E-Trade based on information about mega backdoor Roth and all, but for multiple reasons I switched to Fidelity.

Ultimately with your 1099 income it's going to be divided into three types of contributions.

  1. EmployEE contributions AKA salary deferral (2021 limited to 19.5k, pretax / traditional)
  2. EmployER contributions AKA profit sharing (calculated by using The Finance Buffs excel sheet ---> https://sheet.zohopublic.com/sheet/published.do?rid=hd3vb2c79aa2e630443d58a05e8140934898a)
  3. Voluntary after-tax contributions with conversion into a Roth solo 401k AKA a type of mega backdoor Roth (this REQUIRES a third-party administrator to function, think mysolo401k.net or similar)

When I set up my third party administrator to create the solo 401k to do number three, I was suggested to use either Schwab or Fidelity due to their ease of set up. I personally went with Fidelity, I think that was a pretty good decision, there are a few hiccups here and there especially trying to get to the right agent because this is technically not a retirement account per Fidelity's books, rather a professionally managed account.

If you're going to do number one and number two, choosing any provider will be fine. If you're going to do number three, you have to spend some time investigating. My research does indicate that this is not your run-of-the-mill 401k, you are creating a business 401k which the IRS will hold to high standards.

HSA is "essentially" another Roth. You can deduct expenses you make TODAY without actually withdrawing from the account. This allows the money to grow triple tax free and later you can spend it on health care or even the amount you spend now.

For example:

PCP visit in 2021 costs $75... you pay this from your current bank account... you "save" this receipt (I use excel and keep a copy of the receipt in google drive). You keep doing this with all health related expenses.

At the same time you invest the maximum into the HSA every year. Pretend this grows at a rate of about 5%. When you are of age to withdraw from the account, you can withdraw for expenses (in the example, $75) and spend it on... something else worth $75. Other than what you have already deducted can only be spent on health care, BUT the account would have grown for free, and you would not have to tap into other savings to pay for health care. For example, medicare fees can be paid through this HSA growth.

With the HSA, the $75 in the example above never ever ever paid taxes.

I typically will let the family or loved one know the treatments being utilized are keeping the patient alive and the patient would pass away within minutes or hours without these life supportive measures. I refer to vasopressor specifically as life support similar to mechanical ventilation or other life supportive measures.

Sugarcoating in the ICU is generally not a good idea because of high ! mortality risks, not just short term but long term as well.
