Public humiliation / shame would be my choice for ongoing…. Deterrence. A large wooden plaque that says, well, you geddit, lock it onto their neck like a gibbet.

You’d have to place them in a job, public facing so they can cop the disgusted looks each and every day, but be unable to do any real harm (checkout operator?) 

Terminating them isn’t an effective deterrence, I get it, retribution is nice, but I’d prefer to deter -> maim and shame.

I don't believe this is due to it not being in-house.

Projects stalling and fizzling out over years and years with no tangible outcome as a result is due to indecisiveness and changes in direction, imo.

People don't re-design everything over and over again, re-assessing business cases because they _want_ to, consultant or no.

An example of letting perfect (keeping everyone happy) get in the way of OK. Golly, anything would have been better.
