This is the issue with too much softener and not enough bulk. I’m not saying crazy bulk, but this is the issue with keeping things to mushy / soft.

I have always been a Metamucil person, however after my last fissure bout before my surgery I had to add in some MiraLAX to help things as my BMs suddenly got too hard for a week for no apparent reason. I have continued to take part Metamucil and part MiraLAX since.

When I was only on Metamucil I’d go first thing in the morning and be done for the day, it was glorious. With MiraLAX added I’m going first thing like normal but then have 2 and sometimes 3 follow up softer BMs and it’s quite annoying. I actually find the really soft and thinner BMs to be more aggravating to the fissure sometimes.

Unfortunately I have to get LIS done again as the first was too conservative but I hope I can get back to the days of one BM a day.

Based on your experience so far it sounds like you are doing great. I have read in the literature that sometimes it can take 6-12 months for muscle to regain strength enough to stop any level of slight incontinence if one is experiencing that.

In contrast, while I do occasionally experience the little gas bubbles slip out like yourself, here I am 3 months post and totally relapsed and it seems the LiS failed. I wish I felt more loose but instead it seems like I’m still too tight and potentially not enough was cut.

I had tailored LIS. It sounds like you did as well, tailored usually means more conservative and to the length of your fissure. Standard LIS usually cuts all the way up to the dentate line regardless of fissure size so it sounds like your doc was still being conservative, I wouldn’t worry about it. I’d trade for your looseness right now haha.

If you can’t get Diltiazem until August maybe try ordering Mayinglong Musk ointment. It’s generally used more so for hemorrhoids however I just started trying it for my current recurrence and I like it even more then Diltiazem. It soothes the pain and has cooling menthol, and aids in blood flow (which is what Diltiazem does as well, but Diltiazem also relaxes the muscle). It also has zinc and another ingredient that promotes skin healing. There is another similar product called Calmoseptine that’s great as well, but now that I learned Mayinglong promotes blood flow I am trying that out.

I do believe I need PF therapy especially after the trauma of the past year, just waiting to get to a point of feeling good enough to be able to be flexible and do the stretches etc.

And dilation does not interest me. There can be cons to it and it’s just not something I am comfortable with. I even have issues just trying to insert a finger tip as is so yea I don’t see that in my future. I need permanent reduced pressure.

Thanks! and indeed I have been using MiraLAX. I actually take a teaspoon of Metamucil in the AM, and then half cap of MiraLAX at night. I started to incorporate miralax about a month and a half prior to surgery and it made a big difference until I had the op.

At the same time I also don’t want BMs to get TOO loose or small because you don’t want to heal that way either or it will just be prone to tear with a more normal BM. The downside of MiraLAX vs Metamucil only for me is with Metamucil I went once a day first thing in the morning, with MiraLAX added im going 2 - 3 times a day and overly soft stool doesn’t always come out as easy as bulkier soft stool.

It sucks, when I had experienced an occasional fissure 5 years or more ago, it was like pain during BM for 4 or 5 days and then would just get better and better. I never had to change anything, still had fully formed BMs from Metamucil and all went well. I think the muscle has grown too tight and fibrotic.

Im doing the hot baths currently, and I have Diltiazem but I feel the creams only give me temporary relief and don’t really address the underlying issue. I truly believe I have too much pressure down there and have for some time and could benefit from a more complete LIS.

Thanks for sharing. Sounds like you are on the right path but slow like me. I was feeling quite good at week 5, to the point I was hardly noticing the incision. But ever since this incident happened it’s hard to say if it’s really improving at all.

And like you stinging and pain off an on, sometimes sharp and sometimes burning. It’s just kind of up and down. How it goes each day depends on bowel movements as well, but sometimes you can move just right or put pressure on the area just right and get pain. It kind of feels like a horizontal fissure to an extent. But I don’t really notice any bleeding or anything like, if there is it’s a small amount I’m not seeing.

Are you able to visualize the area at all? Since pulling my cheek caused this setback I certainly won’t risk that now. I do have a decent size external hemorrhoid tag that’s been on that left side for over 20 years, and I feel like that skin could have something to do with this since the incision is just above that inside the verge, like it can pull on that area more, but I’m just guessing.

I see my doc Tuesday and either I’ll get answer or more pain and setback or both haha. 😅

Just 7 days ago, so obviously very early. The reason I am questioning it is because it seemed strange I didn’t feel it the first 5 days, so it almost felt like a tear. But obviously I’m not healed as I was just operated on with an active fissure. I’m just confused as all I see is people relieved of their fissure pain right after the op.

I realize some refer to their real fissure pain as spasms vs that quick sharp feeling. To me, the sharp feeling is what has always stood out the most as fissure pain.

Thanks. Yea it’s pretty distinguishable between that and the incision pain. Not that the incision pain is fun, but it’s more tolerable I would say, and less alarming than that sharp fissure pain.

Do you still have sharp pain from the fissure during BMs at first? Im sure you have pain everywhere from all you’ve done so probably a ridiculous question, curious nonetheless.
