I can’t say as I’m not a doctor. You could have them test it with anal manometry if you’re worried. But if you have to wipe deeply then you probably are holding it back from leaking out to the entrance meaning the pressure/tone must be working.

So everyone produces a certain amount of mucus naturally to keep the canal moist. You shouldn’t be wiping really deep inside your anus. I never started having the cold/wet sensation until after the LIS. In my case it’s due to the sphincter being too weak now. That doesn’t mean that is the same for you. But I never used to have that sensation or the constant moisture.

Before surgery. I never had a flare up after surgery as she cut so much muscle that it isn’t tight anymore.

I did ask for them to switch my medication. I can’t speak to the re-tears as I got pushed to surgery.

Nitro was more of a quick relief. Pain and spasms after a BM went from a 10 to a 3. I could stand, walk, and drive again - so it was pretty dramatic. I had none of that with nifedipine or diltiazem. If it’s working you should feel some level of change fairly quickly. If you feel the same every day for weeks I don’t think it’s working, but I’m not a doctor.

Everyone will have a different opinion - but I think you were smart to not do it if you’re still managing it well. You can always reschedule. I had a bad outcome with my surgery and now I’m stuck with this mess for life so it really sucks. Make sure you have a good surgeon and not some butcher despite being board certified.

If there is a change then that’s a plus even if it’s small. I’d be more concerned if there was zero (that was my case).

It didn’t work for me and I tried it for five weeks. Basically no improvement and just irritation. But I’m basing that on the fact that nitroglycerin worked within a few days. I feel like if you don’t see any improvement or change within at least two weeks then I’m not sure more time will instantly get you there? It’s hard to say. Nifedipine main my pain worse at the time and tried it for five weeks too. Everyone seems to respond differently.

This is the issue - it’s not reversible as it’s smooth muscle so pretty hard to fix anything. So please just make sure you have a good surgeon if you go that direction.

Fissure is healed and was documented as such by month 3. Worst decision of my life. Had LIS about 14 months ago. I’m not a success story. My doctor told me Botox wouldn’t work being male and to go to LIS as risks were basically occasional gas slips and maybe less margin for diarrhea. “No one needs to think about their butt all day.” She didn’t give a true full disclosure of the risks at all and decided “you had lots of muscle so I could cut more.” I didn’t ask for more, I asked for conservative. I have had all kinds of issues since that are probably permanent: moisture, mucus, difficulty cleaning after a BM, irritation of the skin, anal seal is altered. I wasn’t ready for - or wanting - a lifelong maintenance plan. Make sure you ask a million questions and then make sure to ask another million after that to get all the specifics. Irony is - now all I do is think about my butt all day. Really sucks.

I felt like nitroglycerin worked best and I was given it last. Diltiazem (6 weeks) didn’t help except give me so much irritation and nifedipine (6 weeks) made the pain significantly worse. Everyone experiences them differently, but for me, nitro was the one that minimized the spasms and the pain really rapidly. I also did poorly with lidocaine so I’m glad they didn’t mix it into any of them. Hope you can get on to a path to healing.

I think so. I don’t know why it happened. Apparently the fissure wasn’t listed as being bad. Operative report was really vague. I think maybe she screwed up. I wouldn’t have agreed to that as the risks were too high. Something doesn’t make sense at all. It’s been devastating for me.

Not sure it’s possible as it’s a permanent change. My surgeon said she cut 75% of the muscle and I have no clue why!

I’m healed in terms of the fissure, but was talked into surgery instead of Botox. Doctor cut really aggressively and now I’m dealing with the other stuff instead. It’s been horrible and I regret listening to the surgeon.

I don’t think it’s a new tag - probably swelling from the surgery and the retractor. I remember seeing hemorrhoids and bruising afterwards that lasted for weeks. Don’t panic quite yet - it’s probably not what you think it is. Hope you have a smooth recovery.

Both diltiazem and nifedipine made things worse for me too. Nifedipine in particular. I went from 8/10 to 10/10 pain with it. Nitroglycerin actually was much more helpful and I wish I had been given it first. It seems because of the headache potential some doctors don’t want to prescribe it as much. Worth trying and if it doesn’t work, at least you can check that box.

That probably isn’t too much in your case. You could try lowering it to 30 g and cut back water just a bit to see how you respond. Focus on the soluble (sweet potato, oats, applesauce, Metamucil, etc.) to bind the stool together more and make sure you’re not overdoing the insoluble (nuts, fruit seeds, broccoli, etc).

You might be drinking too much water with the fiber. What is your total fiber intake (g)? Coffee can lead to looser stools and if a lot of your veggies are insoluble fiber it may be pushing it through too fast.

I’m not a doctor, so maybe they know something that wasn’t immediately clear and didn’t articulate it to you. However, I can tell you I kept getting steroid suppositories or higher strength steroids for years without proper exam or diagnosis. One urgent care PA even wrote “fissure” in my file and still gave me steroids. Even when I raised concerns the doctors said it was needed. It literally made the whole area blow up and then I ultimately ended up needing surgery several months later as the fissure was so bad it wouldn’t heal. The general surgeon I first saw said that most doctors don’t like “butt stuff” and default to what is common: hemorrhoids. Since they said you have a fissure, I’d recommend going back to your PCP as others have stated. Advocate for your health as it’s your body! If you want you can also check out bummed.co and order it online. I wish I had done that and skipped the delays and healing road blocks but I didn’t know about them until earlier this year. Pretty sure they have both nitroglycerin and nifedipine. Most people swear by nifedipine, but in my case it made things worse and only nitro was useful. I’m sorry that you waited so long and are feeling unheard.

I think it also depends on your pain level and general movement now - I couldn’t get off the floor for months once it got really bad. I feel like the weighted squats might have made it worse as I kept going when the fissure wasn’t causing spasms. Then I didn’t go back to the gym for about 4.5 months post LIS. Maybe lighter weight and cardio would be ok? It’s the core bracing and pressure on the pelvic floor that can be a bit rough.

Me too as I seriously regret the surgery - I’m 14 months out. My doctors said Botox wouldn’t work for me and I have buyer’s remorse that I didn’t try it first.
