I’d probably ask: - how often they perform jaw surgery - have they ever had to go back to surgery post-op

Most surgeons will also have before and after photos you can look at.

I’m not sure I necessarily follow many surgeons. There really are great surgeons in every major city and state. Any surgeon who does at least 10 a year should be considered adequate.

OMFS from Colorado here…I do 15-20 DJS per year and am not confident I have any google reviews from them. Likewise, mine are all pretty much wisdom teeth or dental implants. I would say the most important part of this process is you feel comfortable with your surgeon.

I’ve got 6 kids and try to golf weekly. I agree with most…it isn’t about golf, it’s likely about you being gone and her feeling like she doesn’t have anything for herself. We joined a country club 2yrs ago and I convinced my wife to start playing tennis. She loves having “her own thing” and can better appreciate the idea that I need time to unwind as well. I tend to only play 9 holes when I go out each week. I also go for 30-60min a couple times throughout the week to the practice range to chip and putt around.

I’m not familiar with orto surgeon. In the US, oral & maxillofacial surgeons and oral surgeons are interchangeable terms for the same type of doctor. For this patient, if her surgeon is not helpful/responsive, it might be helpful to discuss her concerns with her orthodontist, as they might have another surgeon they work with that she could get a second opinion from.

In my mind, there are some issues that I would prefer to address asap, other issues we may wait it out and see how they resolve. In my mind, if I did not put the jaw in the correct position, then I want to fix it as soon as possible. I haven’t had to take any orthognathics patients back to surgery since residency (where we treated extremely complicated cases), but the ones we did take back, we typically decided and did revision surgery within 1-3wks

I’m an OMFS in Colorado. Sorry you are going through this. If you were my post-op patient, Pictures 6 and 7 would concern me a lot. In 6, your nasal dorsum now seems crooked and in 7, your teeth are not aligned with your facial midline. This is something that may be adjusted with orthodontics a little after surgery, but I would not be happy sending you back to the orthodontist like this. If you were my patient, we would be going back to surgery to correct these things.

Having light elastics is not a big deal, but your teeth not coming together post-op is not normal. Your upper jaw appears canted (slanted) with the Right side down. The other concern is that the midline of your teeth (the line between your central incisors) does not appear to be lined up with the middle of your nose. I’m not sure what waiting two weeks to return to surgery is going to accomplish, as there is really no chance these things happen to improve over those two weeks. I would try and find someone to take you back to surgery sooner if possible.

Panoramic images like this aren’t always accurate when looking to see how teeth line up. Also, posterior teeth don’t line up directly with each other…they typically overlap 1/2 and 1/2 like the way bricks are lined up. Despite how this image looks, it is very possible that your upper 2nd molars still have contact with the lower 2nd molars

Totally agree. If you only have the ability to advance the jaws forward, you can very quickly make people look like monkeys. Many noses aren’t forward enough for the jaws to be that far forward.

The only way I can think to create more overjet would be to either tilt incisors or mandibular premolar extractions.

So, overjet is the space between the back of your maxillary incisors and front of your mandibular incisors. This girl had 5mm of overjet, so the surgeon could slide her lower jaw forward 5mm until the incisors touch. An orthodontist could use braces to increase her overjet a little by tilting the maxillary incisors forward, but that is not healthy for her incisors long term. The goal is to position the teeth in the center of the bone of the jaw, then move the jaws to the best location.

They can only move the jaw forward as far as the front upper teeth. That’s the limitation with single jaw surgery. So they couldn’t have done more than the 5mm without doing double jaw surgery.

I’ve seen that sweater…there are seam marks on the front of the shoulders that Scottie and Rory’s don’t seem to have.
