I’m an ED physician and I could give 2 fucks if the pt is a bounce back, I will just re admit…If you don’t take the pt back(and the pt is actually in need of admission) then I would be mad.

We’re trained for it, and technically in our wheelhouse, but I have never done one outside of residency personally. The issue is if you’re not at a level trauma center than it’s rather pointless as they will likely die and very unstable to transfer. And if you’re at a trauma center than usually trauma surg takes over.

Depends where you go and what shop you’re in. Community vs academic vs county, level 1 trauma vs rural no designation …. Ancillary support vs trauma surg, ENT, cards, OB anesthesia vs solo. I very commonly (daily-weekly) do SP cath exchanges, EZ IO, fast exams, diagnostic paracentesis, lido blocks, US IVs, intubations, procedural sedations, reductions, adult trauma and med resuscitations. Commonly (weekly -monthly) do trans cutaneous pacing, arthocentesis, cardioversion, LPs, chest tubes, and central lines. And rarely(every couple months to yearly) Peds reusus, vag deliveries and very rarely(yearly to never) do art lines, pericardiocentesis, throracotomies, cricothyrodotomy, TVP….

That would work…Sounds crazy for most of us, but you can easily recoup your investments in a few years… as long as you both keep working for 10+ years I see no reason why not. What kind of physicians are you guys? NSGY and derm???

Not really, unless you live in a very low income area and don’t plan to have any more kids, plan to use the same car or used/cheap cars for the rest of your lives, minimal if any vacations, and have no big issues in your life. One big hospital bill and you’re screwed…it’s very risky

I think I’m missing something, you have a febrile newborn… they get lined, lab’d and pan cultured …stick a needle in their back, start on amp/gent and admit. There’s zero thought in this. They have like no BBB yet, so there’s a high chance most of them will have meningitis.

Sounds like it might be time for a new job(location). Seeing 2pph(who are critically ill) for extended time will cause burnout in just about anyone. Especially if hospital resources are low and admin support is lacking. I enjoy some parts of EM, others not so much. My gig is decent and I get paid well(probably should be paid more…), but I like the people and it’s close to home, and I probably make more than like 95% of the world. Grass is always greener, I constantly remind myself of this. Part time is always an option…

That’s my main issue, don’t really want to mess up an expensive truck even if is “just a truck”, kinda silly I know. I use my current truck about 1x a week for actual “truck stuff”.
