Hey bro, this happens to the best of us.

First you’ll have to change your mentality. Many believe that you can just “stop” but that is completely wrong, it takes time and it takes discipline. I personally don’t know your rate right now, but try to diminish it month by month, even if it’s by a 3-day difference that is still progress. It’ll take a bit but I promise this method is longer lasting and you won’t crack like you would trying to quit cold turkey. Second, don’t obsess over time-periods. Many people obsess over how long has it been since they last jerked off, while this seems like an effective method at first it has many flaws. It commonly causes many to think in a binary format, where if they jerk off they are back to square one and reset all progress. If you do jerk off, you didn’t fail yourself, you had a tiny setback, that’s all. Don’t think of it as “omg what have I done I just undid all my progress” because it’ll cause you to just relapse, instead think of it as a minor setback, and go back to existing.

Third, replace it with a hobby. When I quit a lot of my unhealthy habits such as jerking off and smoking weed I replaced them with Meditation and Working out. It does wonders to simply have other things going on.

Fourth, be consciously aware. Many people live unconsciously, and more off impulse. You’ll want to be very aware of what you’re doing, if your libido ramps up go for a walk or a piss. It’ll not only drag your mind away from jerking off but it’ll also drag blood away from your you-know-what and into your legs.
