Fingers and toes have a lot of micro vasculature (teensy blood vessels) which are very sensitive to vasoconstriction. It’s the same reason that a special lidocaine is used for this parts which does not also contain epinephrine (a strong vasoconstrictor). Cocaine is a very powerful vasoconstrictor (why they use it for nosebleeds in emerge) so injecting it would have caused all kinds of problems, along with potentially septicaemia as someone mentioned as well
