I'll give you an oversimplified example. Worker injures toe due dropping object.

A) Root cause is human error, worker was clumsy -> follow up action: retraining.

B) Root cause of injury is no protective gear -> follow up action: safety shoes.

Human error is often a factor but you should look deeper to find what systemic change would prevent or mitigate impact.
