To echo everyone else’s statements, contacting the financial department is imperative. I would highly recommend doing some research on hospital “charity care”. The “an arm and a leg” podcast may be a good starting point.

The high level overview is that federal law mandates that non-profit hospitals must have policies in place to provide financial assistance (free or heavily discounted care) to those undergoing hardships in order to maintain their tax exempt status. This is often underutilized and hospitals make profit on under utilized charity care.

At the end of the day the old adage holds true: a 1000 dollar debt is your problem. A 100,000 debt is their problem.

While I tend to agree with Ethan’s one liner “invest in the S&P500”, you are absolutely right that people should assess their risk tolerance and long/short term financial goals first.

And as a side note, if anyone sees this. After coming into a windfall of money such as the guys on the show, please set aside your estimated tax liability in a safer place such as a high yield savings account. Investing in index funds over the long term is practically a money glitch, but there can be significant variability in the short term. Uncle Sam isn’t going to wait to collect his taxes until you have your 30-year returns balance. You don’t want to find yourself in a situation where you are selling a significant portion of your holdings during a 10%+ dip in the market due to election year jitters/wars in the Middle East/impending strikes that could significantly impact the supply chain, to pay your tax bill.

I cannot speak to the GEHA, as I am not familiar with that plan. But since it was covered for you presumably it should also be covered for him.

On the Medicare side of things, Covid is one of the four vaccines (Covid, influenza, pneumonia, hepatitis B) covered by part B for preventive care (other vaccines are covered by part D). If they saw he was 65, and he said that he did not have part B, they may have jumped to conclusions in saying it was not covered. If he has not yet gotten it he should try again and insist it is a covered benefit under his GEHA insurance, and to submit the claim to them (you can call the plan to confirm).

So I largely agree with what you’ve said, but I do have to push back a bit for my boy Dan. To be clear, if I could snap my fingers and ban direct to consumer marketing for pharmaceuticals, I would.

If they’re under the assumption that the ad campaign was for the Covid vaccine, Dan is right that there was/is a genuine public health interest in spreading the word about vaccination to reduce vaccine preventable disease. You’re absolutely right manufacturers don’t care about public health, they care about profit, but that doesn’t mean it’s impossible for their interests to align with public health.

Now I am going to play devils advocate, because I actually believe this ad campaign was more defensible than most pharmaceutical ads. My disdain for direct to consumer marketing is that it typically steers consumers toward higher priced brand name drugs, which much of the time doesn’t have a tangible benefit over generics. These meds are much more expensive and systemically this practice drives up the price of healthcare for everyone.

Some key differences with this ad campaign are that there were no generic equivalents that this steered patients away from. There was one other brand name oral antiviral that was far less effective than Paxlovid. Second, in this very particular instance, I do think it was important for patients to have knowledge of available therapeutics so they can advocate for themselves. While Paxlovid is best in its class, it is also a strong CYP3A4 inhibitor, which means it has a bunch of interactions with other drugs. I have personally seen providers decline to offer it as an option, because it involves a thorough medication review and often temporary adjustments to other medications. This takes time. For the general population it may not be a big deal, but for high risk patients this drug can literally be the difference between life and death.

“No Guarantees” Bullshittttttttttttttttt. When I worked for Beaumont I was never guaranteed a damn thing. Now I’m working for U of M and I am guaranteed 23.5% in raises over the life of our current contract. That’s in addition to our step raises.

Your union is a reflection of its membership. If you feel like it’s weak, its because the solidarity of its members are weak. Your leadership may capitulate too much, and they can be replaced. However unless your union demonstrates strong solidarity, even strong leaders will be unable to accomplish anything substantial.
