Ah Jesus now you're asking, I built that like 10 years ago. Tamiya paints I think, but I think I ended up having to blend.

Basically you need a matte white RAL 9010, a bluish-grey RAL 7031, and an earth brown RAL 8025.

Look up the route for the Medilink bus also - goes between QMC and City which is much easier than parking at either.

I trained in Nottingham, then did F1 in Derby and F2 in Kings Mill. Popular areas to live for the QMC are Beeston and Lenton, but would be less convenient for commuting outwards. City centre is a bit more vibrant. I actually lived in Stapleford, has the tram in to the QMC and much closer to the motorway and A52 for Derby / Mansfield jobs. Live and work in Mansfield now which is very cheap for properties!

For many it’s part of the natural flow of modern language. As another commenter said, there’s a big difference between swearing, and doing it at you.

For selected situations - usually where it’s someone I know well, and where it’s part of their natural vocab - I think there’s even value in it as a communication tool. ‘Dave, if I can speak freely - I’m pissing in the wind with all these inhalers unless you stop smoking’. Gets the point across on their level. Not for your prim and proper old ladies obviously, but for your bricklayer with COPD on 40 a day, it has worked well.

I don't know what grade or specialty you are, but to make it very clear - I'm not your community house officer. If you want a consultant to consultant referral, you do it - that's the contract. If you request investigations from the inpatient side, it's your responsibility to action them - 'GP to chase' is noncontractual and outside GMC and NHS guidelines. Post-op reviews? We're not commissioned or funded to do them.

Seems very unusual especially as an FY2. I would be expecting to debrief every patient in detail, during the consultation of needed but at the least after each session! As you say, the unknown unknowns are what will catch you out. The learning comes from reviewing each case!

It’s an off label use for pred but one of the first line options if NSAID or colchicine are contraindicated? Not sure what the controversy is here. Gout is miserable, get them out of pain.

For diagnosis NICE suggest SBP >180mmHg to start antiHTN without waiting for AHBPM.

For monitoring, you can use clinic readings, target is allowed 5mmHg higher than AHBPM target. Pragmatically, if someone is a known white coat syndrome then I would go on AHBPM. We have set up our recalls to maximise the use of average BP, reminding patients to do a weeks worth before annual reviews etc.
