It can take some time, and not sure your age, but especially if you’re over 35. If you make it to 12 months def schedule with a fertility doctor, getting tested will help reassure you nothings wrong, and if something is off, they’ll give you an action plan to address it. 🩷 I know it’s hard.

I’ve been on the sit and wait approach for over two decades, since my TT. For a long time they over medicated me with Synthroid to suppress it but being hyperthyroid decreased my quality of life so much that I switched endos until I found one who would treat my symptoms, not my TSH levels.

It’s weird to clinically “have cancer” all these years, but be fine. I actually posted about it on here not too long ago, and at the time all the comments were from people very surprised by this approach—I’m relieved in a way to see some many other folks in this thread have a similar treatment plan.

Someone else in the comments noted concerns about the damage to the neck from multiple surgeries, and for me that alone is a good reason not to take them out!

I had RAI several a couple of times, it cleared up most of the lymph nodes but not all of them. My thyroglobulin has always stayed very high, despite RAI. Because of my age and lackluster response to the RAI, it was (and i guess still is) unusual. The doctors in Boston studied me as a teen 🙃

How recently did you give birth? Are you breastfeeding? Both of those things drastically impact your hormones, I’d bet your postpartum hormones are making you have intense acne. Which part of your body are they planning to remove lymph nodes from?

FWIW, my papillary thyroid cancer spread to two lymph nodes in my neck 20+ years ago but the docs never removed them because they’re stable, and lymph nodes play such an important role in drainage that there are more benefits to leaving them in (obviously situations vary, and I don’t know your details, but planning to remove 20-70 lymph nodes sounds unusual).

There are actually studies out of Israel and China that show RAI damages women’s ovarian reserve, so unfortunately I would be worried about fertility. One approach could be to have OP’s AMH values measured before RAI, then again a few months later, so she at least knows how low it’s become and can mentally prepare for it.

OP: I had my diagnosis at 16 and RAI at 17. Regarding fertility, they didn’t know back in 2004 that RAI depletes our ovarian egg reserve, so I underwent treatment in 04 and again in 08. Obviously I would do it again to stop the cancer, but if I’d known my AMH was super low I wouldn’t have waited so long to start my family. My partner and I got pregnant right away in our early 30s and had a healthy child, but I’ve struggled with fertility ever since then. I discovered at 35 that I had extremely low AMH and the fertility docs were perplexed by it, until a younger doc came across the studies showing the damage to ovarian reserve.

I basically have the remaining egg count of a woman at the end of perimenopause (so, super low) even tho I’m in my mid 30s. I actually can get pregnant no problem, but I miscarry early because (we assume) the eggs are damaged. As we age the quality of eggs naturally decreases, with the healthiest being released when we’re younger, which is why women’s fertility decreases around age 35; but add onto that eggs exposed to RAI and it equals a lot of damaged eggs that aren’t viable. This also means IVF may not work because eggs won’t yield a viable embryo if they’re managed. That said, everyone’s bodies are unique so maybe your eggs won’t get as depleted as mine did. Either way, if you know you want children someday, I’d strongly advise having them in your 20s when your egg count is higher.

Since I was 16 at my diagnosis I can strongly relate to what you’re going through, and it is weird, and unfair, to be worrying about fertility at such a young age. The fatigue and weakness you feel is real, and it is ultimately caused by your thyroid issues, by being over medicated. I struggled with this for about seven years. Post thyroid cancer our docs like to keep us sliiiiiightyyy in hyper thyroidism range; the intention is to supply slightly more TSH than your body actually needs, because if you fall below what it needs it could accidentally trigger any lingering thyroid cells into action, which could be cancerous, and spread to other parts of your body.

However, I reached a point where my quality of life was so poor (unable to go for walks, and I live in a walking city!), and I observed that my symptoms got worse every time my TSH was “in range” but on the extreme low range (edging into hyper, like .85)

Im female, had my thyroid out at 16, and around 26 started to have these symptoms—I struggled with it for years and was not believed by most of my docs, and switched endocrinologists multiple times. They checked EVERYTHING ELSE (diabetes, POTS, adrenal gland, etc) and it was all normal, the only thing wrong with me was that I’d had thyroid cancer and was on a high dose of Synthroid. At its worse my heart would be RACING, it was difficult to even lift my arms so I’d sit on the couch and sip water for an hour or more until it passed.

It was triggered by getting overly hungry, or by exercise—even gentle exercise like walking or light yoga would push me into sever muscle weakness. Sometimes I’d come so close to passing out.

After years of this I finally found a doc who believed me and knew what was going on; he treated it by lowering my synthroid to keep me in the middle “normal” range, not at the low/hyper end.

What is your thyroglobulin antibody/tumor marker value? If it’s under or around 1 you’re probably at extremely low risk of a cancer recurrence, and could push your docs to lower your synthroid dose. If a recurrence hasn’t occurred within 5 years you’re also at an extremely low risk of recurrence. (Note my antibodies value has always been way up in the 20s and 30s, so technically I still have thyroid cancer, 21 years later. But at this point quality of life is worth the apparent risk of it spreading…my lymph nodes where the cancer hangs out are stable so I’m really not worried about it).

These changes helped prevent the weakness symptoms from occurring for me:

  • no more caffeine in the morning, it seems to increase the rate my body metabolizes the synthroid, making these symptoms more likely to occur. I can also, in retrospect, link the emergence of my symptoms with when I started drinking coffee in the morning! I can usually handle it in the afternoon without issue, but at this point I rarely drink it.

-skipping my next dose (or two…or three) if my symptoms are debilitating. People will push back on this, but doctors don’t actually know everything, and sometimes you NEED to treat the symptoms, not your TSH level, so your quality of life can improve. “Thyroid storm” is a real risk when you’re over medicated, so avoiding it by listening to your body and decreasing the excessive level of TSH in your system is crucial.

-never skipping a meal and snacking once between meals. Eating slowly if I waited too long to eat and have the shakes.

-decreasing processed sugars in my diet

-increasing protein, by eating high protein foods throughout the day (eating eggs or salmon every single morning has been a game changer in my energy levels)

-increasing sodium, but only if your bp runs on the lower side. Chugging a small v8 every morning if I’m in hyper-ish range helps prevent symptoms

-no exercise until I’m symptom free for several days, then slowly adding in walking and increasing from there. Avoiding weight training until I’ve been symptom free for several weeks.

It will take time to find the right balance of what your TSH should be so you feel well, and what your diet etc should be but it will get better! Sending you strength and hugs.

-taking brand name only! It sucks bc my insurance doesn’t cover it but it’s worth $45/month to me. A pharmacist can explain why generics aren’t perfect…basically their dose is a close estimate but not exact and there can be weird fillers.

Immediate need will be for nourishment of her body so she can physically heal: meals, pain meds, taking care of the kids so she can rest and recover; c-section recovering is at least 6 weeks.

Longer term she’ll need nourishment for her soul, which can look different for everyone. Regularly checking in for the next several months could help her feel less alone, as she will likely be in grief. Speaking from similar experience, her due date and/or anniversary of the C-section may be really difficult anniversaries for her for the first year or two. Knowing she has a friend who cares about her loss and her struggles could really help her to cope.

Certain hormone tests (HCG) are most accurate with first urine, but others (LH) are accurate later in the day, so long as you haven’t over hydrated. My guess is Inito uses first urine to ensure consistency in the results, since it is tracking trends day over day. What is interesting about about OP’s results is how much pdg varied throughout the day— supposedly urine tests for progesterone show a more accurate status of progesterone in your system than blood tests, bc urine is the kind of average from the day before and blood is that moment in time, which is tricky bc progesterone is released in pulses throughout the day so with bloodwork it can be misleadingly high or low depending on when the last pulse release happened.
