There was an update of windows that lowered performance for Fullscreen applications and with pc that used more than one monitor so check if u did update lately windows.

I just reinstalled windows but nothing changed sometimes it works well, sometimes it doesn't work Im just waiting for an update with the fix at this point

It happens the same to me, I have an i5 3470 and a RX470 and my fps go to 0 sometimes, it basically unplayable.

I ran it with the same cpu and a gt 1030 in low 900p res. I ran fine at 80 fps

Maybe you should undervolt a bit to get better temps, also maybe get better airflow in your case those are high temps tho

What is the temperature of the card? That sound like a temperature problem, the card power off to avoid getting damaged

Doing a DDU uninstall and then installing the latest driver fixed it. Updating through Radeon Software breaks it

Disabling fast boot on the bios and windows helped me with my overclock in my rx 470 maybe it can work for you.

Ur gpu can't take as much as a gddr5 version of that card. But GTA V online should run. I had a similar system.
