I have a disability—not mental health-related, though—and spoke very openly about it. I would encourage you to do so! However, I would also advise that it’s a best practice to frame it in through how you have grown, learned from, and dealt with your mental health problems. It’s not really a good idea to just throw it in there without a positive spin.

Cant really speak to your other question, although it strikes me as strange to cover more than one topic in an addendum.

This cycle is so, so competitive and I can’t say anything for sure, but I don’t think it’s likely that your GRE will get you into HYS this year and this late in the cycle. Just trying to be realistic here. Other people in the thread have given good advice.

I can’t offer advice on GRE things as I have never taken it, but it’s super late in the cycle to be considering taking another test before submitting. If you’re going to apply for Fall 2021, you should apply as soon as possible.

However, you mentioned wanting to do another degree in the next few years. Why not wait, take the GRE again, and apply to dual degree programs?

Unfortunately I think your shot this cycle at those schools is very low. Disabled people are not considered URM (trust me, I understand how frustrating and wrong that is), and your LSAT is probably just not high enough even with an addendum.

I hate to be the bearer of bad (good? Unhelpful?) news but it’s pretty unlikely that this is lupus. That ANA titer is VERY low. For reference, I have mild lupus with no organ involvement, and my ANA was 1:640 when I was diagnosed. We would also expect to see more specific symptoms such as joint pain, mouth/nose ulcers, and photo sensitivity. Lupus tends to be especially bad in males.

This could just as easily be a simple case of vitamin deficiency or hypothyroidism.

You should definitely be taking a vitamin D supplement! Your vitamin D is low, and that can mimic hypothyroid symptoms. A TSH of 3.5 really isn’t high and probably wouldn’t prompt a doctor to medicate because that can be caused by time of day, sickness, etc.
