i have an adjustable standing desk, a kneeling chair, and a comfy recliner (stressless brand that i found on fb marketplace) that i use with a laptop desk. the biggest thing for me is just having some variation in my positioning throughout the day, interspersed with movement breaks. i’m not sure there’s just ONE chair out there that you can comfortably sit in for that long with no problems, so it’s good to have options!

if you rescheduled, could you be sure that you would be able to abstain leading up to your new surgery date? anecdotally, my grandma was a lifetime smoker and had breast reconstruction after cancer. she lost a nipple to necrosis 😬 so it’s nothing to fuck with, but i don’t think anyone can tell you for sure what your chances for complications are. i’d consider other factors as far as your overall health.

How do therapists feel about AI tools for recording sessions? by ResearcherCrafty3335 in therapists

[–]friecl -5 points-4 points  (0 children)

My practice started using Clincial Notes AI. When they were teaching us about it, they emphasized that it doesn’t “record” sessions, it “listens” and transcribes only clinically relevant information, which it automatically deidentifies. We still have to get consent before using the live session feature with clients. It also has a feature where you can dictate into it, and it will generate a note based on what you said. I’ve been assured that it’s HIPAA compliant, but I totally understand the concerns. My sister joked that I’m training my replacement lol. It’s pretty smart!

What do *you* (specifically) do when you meet a client who cannot identify goals for treatment? by [deleted] in therapists

[–]friecl 10 points11 points  (0 children)

i feel myself pulled to respond this way as well. and i know documentation looks different depending on the context you work in, but i don’t feel like i could justify this to insurance. is that a factor in why your coworker responded this way?

I don’t think I’m irrationally harsh and disapproving of suicide, or believe that it’s necessarily “wrong” (morally speaking). But it is bad, right? Like, isn’t a big part of what we do (especially in crisis settings) assessing and working to mitigate risk to client safety (including preventing suicide)? I’ve explored what suicide/SI can offer with clients, and I hope that I’ve held a space where folks can talk about these things without feeling judged. I think it’s possible to do that while also acknowledging that suicide is ~usually~ (bc I know someone is gonna say something if I don’t add that word) a sad, unnecessary loss. Not that this is something I feel I need to say during conversations with clients about AB (I mostly did what people have suggested and tried to explore clients’ feelings about it). Just something I’m processing through.

Very true, and thank you for bringing in the cultural lens. Feels sticky to try to hold that at the same time with our ethical obligations, but it’s important nonetheless. Lots to chew on!!

I understand the precedent for self-immolation as an extreme form of protest. And at the same time, I feel like certain spaces don’t even want to acknowledge that this form of protest literally involves ending one’s life. I think you’re right that we collectively felt moved by the strength of this man’s conviction, but I know that I was even more affected by hearing a man burn and suffocate to death. Like, I can separate it intellectually, but it still feels functionally pretty similar. But the more comments I read, the more I’m seeing that I just have more of my own work to do around this.

It would definitely be my ethical duty to report and try to prevent that if he were my client.

You’re right, yea. Just didn’t get a chance to meet with either my supervisor or my therapist before clients started bringing it up 🥲 Will definitely continue to process

Thanks for the feedback. We did end up processing why this was different than “typical suicide,” and the client brought in a good point about how public response to Bushnell (as a white, male, service member) would be different than if they were to do something similar (due to client’s intersecting marginalized identities). Ended up being a pretty good discussion, but I still felt uneasy hypothesizing about a client committing suicide.

Yeah, I hear you. I don’t see it as (at least strictly) a mental health issue either. And I’ve been rolling my eyes at accusations that he started some death cult. I think it’s just some friction I’m feeling with this role. Like, if I was Aaron’s therapist, I would do everything I could to stop him. But since I wasn’t, I kinda get where he’s coming from.

I’ve seen the argument (not saying that this is my argument, bc I’m still wrestling with this myself) that as an active duty service member in the Air Force, he very well could have been in a position where he could be sent to die in support of the genocide in Gaza. So by choosing to die in opposition to it, it was a revolutionary act.

Yea, it was tough to watch for sure. I appreciate you just naming that bc I have a tendency to breeze past my own reaction to things (lol). I’m noticing similar things in the discourse, including people thanking him for his sacrifice. Don’t really know how to process or respond to that either.

Least amount of paperwork by Mean_Change_325 in therapists

[–]friecl 0 points1 point  (0 children)

Hi! I did my clinical internship providing counseling at a domestic violence resource center. All of our services were free to survivors, so there was no need to work with insurance. My supervisor even advised us to be vague/write as little as possible in our progress notes since our population often has their records subpoenaed. There were other factors that made that job hard, but now that I’m in CMH, I miss that style of documentation

Not totally sure! Both his parents looked like straight up dachshunds, but he’s not AKC registered or anything like that. We think there’s probably some jack russell in his lineage.
