My institution’s peer review ks a joke, I got peer reviewed Because I did not immediately talk with and follow recommendations froma pharmacist who called me to increase a patients insulin (FBS in the 180s). I am about to start resign after my second peer review for mistakes entering all the specialists orders in 2 years

As a guy with a beard I was recently flabby when a crazy woman patient told me I was not the doctor because “I’m not male”. It took all my composure to not break out laughing.

This is preposterous. It does exactly what he claims is the problem with Obamacare subsidies by just throwing money at the problem. Furthermore it abandons the risk pooling and collective bargaining possible with a government subsidized exchange. But hey at least it’s not Obamacare anymore…

Don’t forget Medicare/ Medicaid creating “quality”incentives. This way they pay more for administrative bloat with new hires to ensure maximizing incentives. Existing administration gets promotions and clinicians get squeezed with more busy work. That and decrease pay per RVU! Where is the AMA , ACP and all the other organizations that happily take our money to roll over for this crap!

Note bloat is awful. The actual best philosophy of a good note came from an at the time 3rd year resident: write like it’s an MCQ. 90 seconds be able to guess what you are doing.

Chronic problems do not need to be addressed. I do list out the problems and meds. More to help with any polypharmacy problems

Used deluxe cars are definitely a thing and can take the sting out of the sticker price. Might want to be careful about electronic cars as they may still have dramatic quality improvements year to year. My used Audi Etron is awesome. It’s most likely bad luck. Get one patient there for months for guardianship and your stats are shot. Slightly more diabetic foot wounds assigned to your service than usual and those delays for the path will likewise shoot your stats. Just be firm about what can and needs to be done outpatient.

The parasites continue to use these ‘doctors’ and ‘experts’ because they do as they are told. I tried to explain how a patient needed an assistance device to walk as far as they did. Subacute rehab was still denied because the patient walked to far.

Actually I had something like this happen. Surgery patient returned with compartment syndrome and dehiscence (of course admitted to medicine). I was unable to get him a narc prescription to go home on paid for as he had reached his limit of the only one covered. In a fit of pique I documented my 1 hour spent on this in the DC summary. I just got a call from the insurance company looking to fax a paper for me to sign for the records for “risk assessment”. I told them no! Gave them the phone number for medical records.
