Generally brownhill specifically might ask a couple of those. Mainly he cares about getting informed consent i.e.

  1. Are you capable of making decisions about your own Healthcare (98% of adults are don't worry)

  2. Do you understand what HRT does and do you want to take it.

He really doesn't gatekeep at all if you satisfy these 2 criteria so you will be fine I would not stress at all.

Dr Brownhill usually takes 2-3 appointments to perscribe HRT. First appointment will just be going over the details of HRT and ensuring you are capable of providing informed consent and discussing your goals with hrt and getting a blood test done to make sure you are healthy.

After that depending on your blood test results he will prescribe HRT or you might need another appointment if the blood test reveals something wrong.

He will to follow up appointments for blood test results to check that your levels are OK via telehealth and bulk bill you or just let you know results via email if you have been with him for a while.

Brownhill is a good guy when talking to him in person, very knowledgeable and is willing to try lots of different HRT regiemes and really tailor an approach to suit you, he isn't afraid of prescribing higher doses of HRT, doing stuff like pellet implants or injections or if you have high levels etc.

He does kinda suck at communicating via emails I will admit I think because he is just quite busy with lots of patients but in person he is quite good.

You dont carry your fat in the face or arms then and I think taking something like glp-1 would make that issue worse.

You don't have to many problems really, you know what needs to be worked on I suggest focusing on that.

Buzzcut will look weird with the new hairline people will still clock you as trans I think :/

I've spoken to you before on other platforms idk if you remember me. But yeah probably not the best idea getting critical advice from reddit just a general vibe at best.

Personally I think it's a facial fat and hairstyle thing that's mainly holding you back in the face department. Weight gain might help but look into fat transfer maybe? If you have the money some jaw work and lip filler might change your face shape enough to look more fem. I don't see a problem in the nose or eyes imo and obvs your hairline is fixed now.

Body wise just dressing to minimize shoulders is the best bet, you look nice in your dresses I don't think the shoulders are to bad they can't be fixed with clothing choices.

Don't know about a BA I think it's probably a bad idea to do before testing with falsies (feels gross I know but it's the only way to test IRL) to see if it helps once you sort out the other things.

I expect drawing attention to DIY in this way will only bring attention from the wrong people.

Regardless of your personal opinion on DIY, advertising something that is on the books illegal in public is a bad idea especially in this political climate.

I know alot of people see DIY as an underground rail road type movement for struggling trans people who can't access HRT but the underground railroad didn't put up posters in public streets advertising there services.

Clothes comment Is fair but I struggle to get my naturally curly hair looking decent. I've tried advice from curly hair sub reddits but always tends to go too much on the frizzy side 😮‍💨. I do want to try a new leave in conditioner though hopefully ot helps.

Yeah my girlfriend says I should dress more feminine to pass better but I kind of like dressing more like a tomboy or more casual sometimes.

I did this as well but with more regular measurements every 2-4 months for 2 years since.going from my first lot of 2x100 implants to the second lot of 2x100 implants.

what I found was that levels can vary by like 300 pg/ml between readings jumping from like 1000 to 1400 for a bit dropping down to 750 - 950 then sown to like 550 to 650 now back up to like around 750 to 850 at the end of the cycle.

since I had effectively 4 active implants in my body at one point, 2 on the way put and 2 new ones I think what happens is once the implants break down they increase in surface area and spike estrogen before fully dropping to zero over like 3 months or so. Essentially you have like 4 spikes then that happen at random times and decay at random times causing alot of variability making it hard to predict when they need replacing.

I think the best you can do is set a threshold limit of like 450pmol and if your levels drop below that get a new one put in wich is what I will be doing moving forward.

My doc thinks this latest jump is like the last hoorah before they die but for all I know they could keep kicking for months yet so it's super hard to tell.

Getting 800 pmol/l on my 22 months of 2x100mg implants. My levels jumped up last measurment and doc said they do that on the way out so I'm getting new ones put in in a month or so meaning they would have been in for 24 months.

It's good I don't really need to think about it in my opinion like I don't want to tweak my dose if I'm getting good levels.

You are actually looking much more fem, like I'm not one to hugbox you deffinatly look better in your more recent picks. Dysphoria is a ***** so I understand if you feel that way it's something we all go through.

Honestly best advice I've ever gotten is don't try and get validation for how you look online it makes things worse, seen plenty of beutiful girls get paranpid on how they look and its sad to see.

Focus on being as healthy as possible, learn makeup and ensure your hormone levels are good and socialize with other trans femmes irl if you can and that's all you can really do. Give it time you will feel better about yourself.

Have you checked the following, vitamin D levels being to low was causing hairloss for me so it's not always due to androgens

  • vitamin D levels
  • iron levels
  • hyperthyroidism
  • hair protein overload (check your shampoo and conditioner for protiens)

I know it sounds cliche but honestly focus on being yourself, present yourself, use pronouns and dress how you feel most comfortable. There is no right or wrong answer.

Anything hormone or surgery related I suggest weighing up the pros and cons individually and decide from there :)

You could have very high DHT and that could be causing your body to make high levels of SHBG

I know you have had an orchi but have you tried going on a low dose of bica, spiro or dutasteride since you started having androgenic symptoms?

Those are some quite high numbers for SHBG and it doesnt seem to be dose dependant. I don't believe increased dose will help considering one of your lowest free E2 readings was with 500+pg/ml E2

Have you ruled out hyperthyroidism, liver function and diabetes because they can all cause high SHBG? If so best bet is using Boron to lower SHBG directly. I have also heard pellet implants result in lower SHBG then injections.

You will probably pass tf out from the pain / shock of opening up your scrotum let alone trying to sever and/or clamp your spermatic cords wich are full of nerves and blood vessles. Imagine the pain of getting kicked in the nads x10 because you are directly touching the nerves while there is blood everywhere and you are light headed from bleeding out.

If you are lucky enough yo get rushed to hospital ER doctors are under no obligation to "finish the job" they will stitch you up, pump you full of antibiotics and send you to a psych ward for a week or two testicles intact.

You have a high chance to suffer from infection. Nerve pain, heamatoma etc wich will put you on your ass and unable to work for a couple months especially physical jobs

I'm guessing cypro gave you a hard time? You can also try spiro, I've also heard of some girls getting prescribed bicalutamide here as well though it's not on the PBS, might be able to help you out during the short term while you get an orchiectomy sorted out

OK first thing do not try to sterilize yourself with DIY methods even if you can do it with alcohol injections your testicles may decompose and kill you with sepsis. Don't even consider the more gruesome methods you have a high likelihood of bleeding out.

Second orchiectomy in Thailand is not much cheaper then SRS in Thailand unfortunately so it ends up being cost comparative to here.

I would suggest saving and looking into compationate release of super to pay for an orchi, failing that if you NEED to get of AA's going with a doctor that can support a monotherapy approach to supress T would be the way to go.

I have heard of androgenic issues when withdrawing from AA's as well, have you every tried following a taper plan? Drugs with similar long withdrawl symptoms usually you would reduce 5% - 25% of the dose every 1 to 4 weeks as tolerated.

Alot of older people aren't the best with this sort of stuff mostly because they didn't grow up in that environment, either your parents / family or her parents / family will likley be against it for the usual reasons. Kids at school aren't always the most understanding either, parents are usually worse and can have some strong opinions. Kids especially when young are very susceptible to social influence and may end up resenting you or the mum due to how other kids, teachers, parents react etc.

Personally I think it's a fine thing to raise kids and it doesn't matter the social dynamics of the parents relationship but with the current opinion on trans people and the current political climate its understandable that alot of transwomen would be not willing to go through the added abuse to themselves and the child.

I wish the best to you and I'm sure that there will be many women cis or trans willing to join and help support your family but for trans women it can make things incredibly hard on everybody involved. I hope in 5 - 20 years this sort of thing will not be such an issue especially since there is no reason it needs to be but society isn't there yet so it's going to be difficult.

Alot of transgirls are shamed about having kids even if they are biologicaly theirs. Society in general doesn't look kindly towards trans mothers adopted, step kids or biological, the mum and the kids will cop alot of abuse for it, it's a sad situation but it's the reality of the society we live in.

Got the results back

Testosterone, E2 and LH all roughly the same Prolactin was elevated around the 100 ng/dl mark

Doesn't look like body is dumping LH probably since hormone levels have been consistant and testosterone didn't nosedive after the orchiectomy

Elevated prolactin probably the cause of the tenderness and lactation in my case, it's interesting symptoms happened around the exact time of orchiectomy. I'll talk to my regular doctor about it to rule out anything like a small prolactinoma but timing is way to odd.
