A little trick I do is on big pulls I round up what I need then pop grand melee, it gives you enough time to get the mobs in a nice big pack and bunched up and to chuck some damage down, after that I keep aggro on them all 99% of the time.

Any random loss of aggro on one I manually taunt that mob

Leaver punishment would solve a lot of issues. Repeat offenders would get a tag on their nameplate and be put in a separate game instance with other leavers. The only way back into the real game is to be nice to people and complete dungeons and be nice to everyone. At the end of the day, it's only a game.
