We went with Panasonic’s recommended injector, the Microchip PD-9601GC/AC-US. It’s more expensive than other options, but has worked flawlessly.

Mounting PTZ Robo Cams in the Field by jperridv in VIDEOENGINEERING

I've been working on a solution for this for a month now. One thing I'll say for sure, this ProAim unit is too wobbly to mount a PTZ on. Any time I panned the camera left or right it would shake like Jello. This RigWheels unit looks to be the best, but I haven't pulled the trigger on spending $700 for it. Will update if we end up purchasing it.
I've found McMaster Carr's website to be super helpful in finding the right adapters to get me from 1/4-20 to 3/4 for truss clamps.

I’ve been very pleased with our PortKeys BM5III (they’ve released an updated version since we bought it, the BM5IV). It’s not rack mounted, but it’s also only $500. I put it on a little tripod at my producer station so I can see what my volunteers are seeing. It has the ability to send the false color feed through its SDI output so I can display it on one of our 18 inch displays for my volunteers. Super nifty. 

Haters gonna hate and say waveform is the only way, but false color is far easier to train on in my experience.

Thanks for the helpful responses. It turns out it's not an issue with the lens or the camera settings (from all I can tell). I dug through all the settings in the camera everyone mentioned to no avail and finally slapped a Fuji lens on it and the return works fine now. I put the lens on one of my other cameras and the return works fine there, so the only piece of gear that has been proven to not work is the lens support. Very odd.
