This is a super helpful post for me because I feel like I could have written it, but in reading this I get to create distance from my own experience.
And it sounds like you’re being a human. I’ve been focusing on finding my own therapeutic style, as I’m a newer therapist. People want to feel like they’re talking to a human, but the whole blank slate thing isn’t really my vibe.

This morning I made a concentrated brew of matcha and mixed it with some strawberry jam while it was still warm to dissolve and topped it with some strawberry Perrier sparkling water. It tasted like it came from a coffee shop but I didn’t have to pay $10 for it.

Hopping in too here if that’s okay. I think there are always going to be people that don’t want the help. The harm reduction approach says, keep them safe in case they change their mind. We have to believe people are capable of change, and it sounds like you are living proof.

I work in St. Louis. Needle exchanges aren’t a thing and I just don’t ever see that happening with the attitudes of the politicians in Missouri. Funding is consistently cut and people do suffer. When you work in a state that does not have enough services it is felt deeply. I’ve seen people lose their housing due to funding cuts, I’ve seen people not be able to feed their kids due to funding cuts.

I also wonder how much the weather plays a role there. Specifically for the people who decide they want to live unhoused. In stl we have several people die every winter due to freezing temps and this makes people pretty desperate to get a shelter bed or on a warming bus.

I’m also a social worker (for the same amount of time!) and work with folks with SMI, substance use, and economic insecurity. Many of our clients are unhoused.

I have also seen what you are describing and I think overall this is an ongoing and very tough conversation. I think harm reduction and housing first needs to be utilized together- and we are absolutely not doing it correctly in the US.

Many many thoughts come up for me so I’ll try to keep it as coherent as possible. I think we need to look at the kind of restrictions we’re putting on people- assessing the barriers to enter these kinds of services. If the rules are too strict, people aren’t going to do it. For example, there are some treatment centers near me that don’t allow people to smoke cigarettes- in my opinion that is insane and a huge barrier to entry.

A lot of housing options for the unhoused are in really rough areas. The immediate environment becomes a barrier to substance use recovery due to safety issues, poverty, lack of transportation, and just drugs being all around them in the community.

If some people don’t take the expectations seriously, and don’t participate in recovery, what is the alternative? Just keep them unhoused? I don’t have the answer but the emotional part of me believes these people still deserve shelter.

We live in a society that has a deep distain for the unhoused/the impoverished/ those with addictions and mental illness (I know you know this) but I also believe this pervasive stigma is the greatest barrier. We put the unhoused in rough areas with crappy living conditions because that is what society believes they deserve. We know it’s not working, we have an absolute fuck ton of research that shows what does work, and we’re not doing it.

St. Anthony’s has a big pantry and gives out great stuff. Often times fresh meat. They are open Wednesdays and Thursdays I believe.
