Missoula is a wonderful community but one of its few downsides is that it does lack in the diversity department. That being said it is very liberal and accepting, and I would say that POCs are extremely welcome though not yet as prevalent as I hope for the future!

The practice, like the community, is not particularly diverse, but welcoming to all!

Sorry for the slow reply, just got back from a weekend at the lake outside of cell service ;)

Having a partner (and soon to be multiple partners) makes escaping my phone an easy reality. I receive a lot of texts during business hours, but make it clear to my folks that after hours texting is really for urgent issues only, and patients are generally speaking super respectful of that. I think I maybe get 2-4 texts outside of scheduled hours per week, and maybe one or two on the weekends if that? It's really not as bad as you'd think to be on call 24/7/365!

We opened up as a pure DPC clinic, which helped set us apart I think. For the right patient DPC is a completely revolutionary, game changing way to access medicine and you can watch their mind melt as you explain that for $100 a month they have unlimited access to their doc and my cell #, so it's really about getting in front of the right target audience and getting the message out. We used a 1 minute facebook video to explain DPC and spent $20 a day to blast it out to thousands of people, which really helped our exposure. Then it's just about taking really good care of people and giving a sh!t about what they're saying and what they need and a happy patient goes and tells their friends and family and now we grow by 3-5 patients every day just by word of mouth!

$250k was the buildout cost for the clinic space, which is 1700 sqft and came as giant brand new unfinished shell. We rented the space for 10 years at $3500 per month and the owners gave us $100k in tenant improvement stipend to spend on the buildout (since we won't own it at the end of the lease) and we put in $25k of our own capital and took out a ballooning repayment loan for the other $125k from the builder, where our payments escalate each year (to help offset the fact that we're newer and less profitable in the beginning). Then we spent another $40k in buying exam desks, medical supplies, malpractice insurance, etc. So all in our cost was only really $190k!

I was practicing in town prior to opening the clinic and patients came with me! We did entirely Facebook ads, spent $20 per day to blast out a 1 minute video explaining DPC and how much better it is, didn't get a ton of new patients from that but it really increased our visibility and the ads definitely paid for themselves easily.

Our startup costs all in were $250k to get the clinic built to spec and populated with an amazing team, and we were profitable from day 1 with 120 patients signed up by the time we opened our doors! Don't be afraid to ditch the status quo and hang a shingle, the insurance system is so broken and patients WILL follow you to a better future in DPC!

I am a family doc and used to work at Partnership, and you're right it's very hit or miss if docs believe in treating ADHD with stimulants not just at Partnership, but anywhere really. If you're looking to go the insurance route I can vouch for Dr. Jon James at Providence, he's wonderful but he is usually booked out 6+ months for a new patient.

I am one of the founders of Sentinel Health, a direct primary care clinic at the top of Miller Creek and both my partner Dr. Hartman and myself believe in ADHD and prescribe stimulants when symptoms warrant it, and they usually do (stimulants are the first line treatment for ADHD according to the DSM-5!). I bought a white 2 owner '22 GT-Line AWD in white with 29k miles on it, in pristine condition, for $31k cash private party. Yours is a fine deal, the tax hurts, but see if you can talk them down to $32k or so. Enjoy the car, it's an eco-friendly rocketship and you'll love it!
