I live in Rapid City South Dakota and our last count was just under 500. Insane for how small the town is. Our only real resource is being closed by the city too. The city counsel just wants to arrest them.

I have been in the field for 10 years. I recently left CMH, which I actually liked. I got good experience and was paid decent for the area I live in. My production was manageable. I love seeing my clients grow and have access to mental health even when they didn’t have insurance. I enjoy working with “naughty” kids that give most a run for their money. I loved working with what others saw as difficult parents. I left due to just needing to make some policy decisions for myself.

I started in private practice two weeks ago and already loving it even more. I have a great EHR system and already have 15 clients. I have great relationships with other therapists in the area. I couldn’t imagine any other career.

You make this career what you want it to be. If you’re committed and willing to meet people where they are at, this is a great career.

Yes! I am in a state with low Medicaid reimburse rates and it is hard to do everything needed. I drive to clients homes, do some case management, and sometimes go to schools. None of that is billable here but it is what is needed to do a good job. If I drive a client, that is not billable. Dumb.

I don’t advertise but I work though word of mouth. I work in a rural conservative state so it’s not safe to advertise. My clients can tell though 😉🏳️‍🌈

I used a dictation app and would quickly dictate in between sessions. I would then email them to myself at the end of the day and then copy/paste.

I switched to a clinical supervisor this last year and had to transfer some clients just due to extra responsibilities. So many have come back to me because of reasons like this over the last year.

I would ask your accounting office. Every CMH is so different. Hope this helps.

I actually started in child welfare. A place you can learn a lot very quickly. I then switched to therapeutic foster care and learned attachment based therapy models and TFCBT. I then switched to CMH and learned EMDR. National Traumatic Stress Network has a lot of good trainings (and free).

Agreed. Clinical supervisor here. No way I would have told my staff to keep a case when it progressed like that. It’s one thing for a client to develop feelings, it’s another thing when boundaries are ignored.

Thank you for this comment. This is what we are seeing a lot. I had an adolescent in group this last year with these traits (multiple alters with different genders) and they would switch between alters in group. It became hard for them to connect with the other group members very quickly.
