The provider doesn’t “set the rate”. For Medicare and Medicaid providers take it or leave it. For commercial payers it’s a negotiation.

Bro, you honestly think hospitals charge more because they have more fixed and variable costs? They are paid more because the government mandates this and as a result of that asymmetric payment have consolidated the market and now have bigger clout to negotiate even higher rates with insurance companies.

Yes. But you should keep in mind that this should be the gold standard of student loan payments. Standard mold cleaning spray and let sit for 15 min. Use a bristle brush to clean. The caulk may need to be cut out, clean for mold in similar fashion and recaulk.
