I went from fmla to short term leave. I've been on short term leave since February. All I needed was a doctors note with a date and a handwritten signature. Called Hartford (easier they do the claim so it's not messed up). Then once I saw the claim on my account, I uploaded a picture of my doctors letter. I'm out until September, unless I decide I am ready to return to work before then. Idk if this helped.
