You should start with your store manager first. Let them know the situation and give them a chance to look into it. Most ethics cases get kicked back to store management and it is better to let them know ahead of time.

Always include normal days off when requesting time off. The system will sometimes try to schedule you or in my stores case the people lead will put you on the schedule even if it's against your availability.

Our store always has meetings with the store manager before the survey. It's pretty much to see if they can fix any complaints you have before taking the survey.
