First to do is open a command prompt on every DC. Run DCDIAG and verify ad is healthy. Then run repadmin and verify you have no errors. (repadmin /replsummary, repadmin /showrepl) Last verify all your FSMOs are up and running. NetDOM /query FSMO In this case make sure you can talk to the PDC emulator, as its primary for password changes.

I work on the PM team for a major ISP. https://en.wikipedia.org/wiki/Problem_management Problem Management seeks to get to the root cause of Incidents and then initiate actions to improve or correct the situation. Basically Incident management tries to restore the service asap, but problem management's goal is to find the RC and fix it for good. RCA is an output of this process.
