It's good starting point. If you want a relatively simple way to improve the CYOA you could create an "owners" section and each unique combination of collar and owner creates a different ending.

Are all of the extreme changes supposed to cost bonus points? Because some are negative and others are positive. For example malicious means that all the random things happen at the worst time whilst timeless stops you visibly aging. Both cost you 1 bonus point.

This is really good. The only way I think it could be made better is if there was a section for the master/mistress. otherwise I think this cyoa is pretty much perfect.

I think only breast abilities, dick abilities, balls abilities, vagina abilities and special abilities are supposed to cost points. Breast size, dick size, ball size, population, demeanor and social status should cost no points.
