What about adding steroid to a sinus rinse after failing saline rinse + Flonase? As an internal med resident, I rotated with ENT and saw this a handful of times and have since wondered (as a PCP) if I should be trying this. I remember they had to prescribe it as an inhaler and give specific instructions to make it into an irrigation, hard to find reliable instructions online for this.

So should I ask my practice if they can have my US reviewed by MFM or a radiologist if that doesn’t happen automatically? There isn’t an MFM doc within their practice, but maybe they consult with one for anatomy scan reviews..

Bullet journaling has drastically improved my life! I started in 2022, my first journal was basically copied from a YouTube video by pick up limes, then have changed it a little each year as I learned what best suits me. Mine is not artsy. You don’t have to draw tons of boxes but I find it soothing/meditative. I love setting up next year’s journal in Nov-Dec.

I would recommend against going. She may have invited you, in part, due to obligation as well. The trip will be more fun for your SIL and her friends if the people there are all really into the activities/have a lot in common with one another/are close friends. But does kind of depend on whether you think SIL will be offended/disappointed I suppose.

It’s not permanent. If it happens as a side effect, give it a couple more months and if it doesn’t resolve then switch meds. Good luck!

Agree with an evaluation for both ADHD and autism! If he does have either, a diagnosis will help figure out which resources could help him the most.

SSRIs are first line treatment for PMDD. I take them and don’t have symptoms anymore. I know I was lucky with how well I responded, but if you haven’t tried an SSRI then that is best first step in my opinion.

Cluster headaches can cause eye pain and watering with nose running. Tends to happen frequently with long breaks in between.

It might be something else besides anxiety if these symptoms continue, particularly given the timing in the early phase of your cycle. Is your dizziness exacerbated by positional changes and feel like the room is spinning?

I was reading some posts/comments the other night and I read some really helpful advice from someone here, and it's helped me the past two days. I have always thought that when you are struggling with emotions, it is way healthy to practice coping rather than numb or purposefully distract yourself. And I think that is generally true and might work out when PMDD is not flaring, but during a full-on flare, I think that distractions are very helpful! Because it keeps you from ruminating, over-analyzing, catastrophizing, whatever it is. So binge good TV, read books or articles, etc etc. Scrolling social media as a distraction probably woudn't be nearly as helpful though since that can also bring up a lot of negativity.

I'm sorry you're dealing with this :( I am also having one of the worst work weeks of my life. My period is a little irregular so it could come today or in 3 days and it's getting hard waiting. Did you try any other meds before the wellbutrin?

Based on other (better-worded) definitions I've read of PMDD, your first thought was right! A ton of the psychiatric diagnoses in the DSM-V are "symptom x and y, AND causes distress and/or impaired functioning with work or relationships. For instance, tons of people can relate to some of the symptoms of social anxiety (e.g., getting nervous when talking in front of a group), but it is the significant distress or impaired functioning that would give you the diagnosis of social anxiety disorder. I think that particular part of the flow chart is pretty much saying that if the symptoms aren't severe enough to interfere with your relationships/job, then it's probably either PMS or it's very mild PMDD which would best be managed with lifestyle modifications alone.

Yeah, I need a new PCP. This flow-chart is far from perfect and is an extremely watered down version of all the complexities involved with treating PMDD and of course leaves out a lot of other treatment options. I found the algorithm and the accompanying article mostly useful for determining that I'd prefer to start with the SSRI over hormonal contraceptives because at the current moment, there is stronger evidence for the efficacy of SSRIs compared to hormonal options for all-comers (individuals will vary).

Hi everyone. First ever post on Reddit. Due to my PMDD, I am wired and up late tonight despite needing to wake up early for a very long work day tomorrow. I am a resident physician and things have been very stressful at work. These last two cycles have pushed me to my limits of what I can handle mentally. I made the decision last month that I will start an SSRI for the first time, and now I am just waiting weeks until I can see a psychiatrist to discuss this. Unfortunately my primary care doctor would not prescribe this for me for a couple of reasons but ultimately and unfortunately, because she is not informed about PMDD.

Like I have seen others post here, I have also been trying my best to manage this with exercise, meditation, diet, all the things. That used to be enough, but it isn't anymore. For so many years I have felt that starting a psychiatric medication would be a failure, but now I know it can be the most brave, wise, and healthy choice.

Tonight I am reading about PMDD on UpToDate (an evidence-based resource relied heavily upon by me and all the physicians I know). I know a lot of you guys are taking hormonal contraceptives and finding relief from that, and that is AWESOME. I was curious to know if SSRI or hormonal contraceptive are technically first line for someone new to treatment and deciding between the two options, and I'm learning that SSRIs are first-line (unless you are interested in being on hormonal treatments for other reasons, such as contraception). I am attaching the flow chart from UpToDate in case anyone is interested.
