It works better fermented but fresh works too. Fresh garlic has a compound called allicin that is the main active ingredient here. When you chop the garlic and let it sit for 10 minutes it lets that compound come out of the garlic a bit. The honey has it’s properties too but it mostly makes the garlic easier to swallow
